Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The George E. Wahlen Department of Veterans Affairs Medical Center seeks to construct a parking structure which will be located on the northwest corner of the VA Campus at 500 Foothill, Salt Lake City, Utah, 84148. The structure will replace the existing on-grade visitor/client parking lot. Size of the site is approximately 2 acres or 93,000 sq.ft. It is anticipated that construction will be completed in the first quarter of 2012. The new parking structure will add approximately 120 new parking spaces on an upper level supported concrete slab. The upper level of parking will be covered with a roof using steel construction methods. The on-grade lower level of the parking structure will contain approximately 300 parking spaces. The project includes 2 elevators and one stairway. The parking structure will utilize 90 degree parking with 24-'0" wide two way traffic aisles, ramping will be internal to the structure within the parking areas. Concrete spans will be 62'-0" wide, utilizing post-tensioned concrete construction; no other system will be considered. This project will include a new campus access road from Foothill Boulevard along the west property line south to existing campus roadway systems. This project will also include a new elongated circular drop-off area for patients and visitors along with covered pedestrian walkways at the front of the main hospital entry. Priority will be on a competitive (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; (3) or 8(a) Set Aside basis. The Government will use responses to this notice to make the appropriate acquisition decision. The estimated construction range is between $5,000,000 and $10,000,000. NAICS Code 236210 is proposed for this acquisition. This project will be the first of several subsequent phases in the development of a three level parking structure. Phase 1 will include footings, foundations, stairways and elevators systems that anticipate and provide for the future construction of the subsequent phases. Only Phase 1 will be bid and design/constructed at this time. This project will be delivered using a design -build contracting method. Interested and qualified sources that fall into the above mentioned categories are invited to submit their Capability Statements, no later than April 26, 2010, consisting of appropriate documentation, literature and references that include current point of contact and telephone number. Responses must include (1) recent and relevant Federal Government construction experience within the last three (3) calendar years for projects of similar size, scope and complexity to the project proposed in this notice. Include contract number, location, scope of work, dollar amount, timeliness of performance and customer satisfaction; (2) work in a Hospital or Medical environment; (3) proposed subcontractors to include same information as (1) above; and (4) if submitting as an 8(a) firm, proof of SBA Certification, servicing SBA office and point of contact for the SBA. All prospective contractors are required to have an active registration in the Central Contractor Registration (CCR) and FAR Part 4.12, prospective contractors must complete electronic on line annual representations and certifications (ORCA) databases prior to award of a contract.
