- April 8, 2010 In Reply Refer To: 69D/GLAC PLATT Construction, Inc. Attn: John Lucht 7407 South 27th Street Franklin, WI. 53132 SUBJ: Notice of Contract Award - Contract V69D-C-1398, Provide all labor, materials, tools, equipment, and supervision necessary to construct the SCI Connecting Corridor at the Clement J. Zablocki VA Medical Center, Milwaukee, Wisconsin. All work will be accomplished within 365 days after receipt of the Notice to Proceed. Acceptance is hereby made of your offer of $1,380,315 in your bid of March 31, 2010 Contract No. V69D-C-1398, and Purchase Order 695-C00568, has been assigned to this award and must be referred to in any future correspondence concerning this award. You will be required to furnish all labor, materials, equipment and qualified supervision to perform work in accordance with all applicable VA construction specifications listed in the statement of work for this project, drawings, applicable codes and the latest acceptable standard of professional practice for work of this nature as set forth in the Proposal in accordance with specifications for the consideration of, One-Million, Three Hundred-Eighty Thousand, Three Hundred Fifteen Dollars ($1,380,315) and to complete work in Five-Hundred and Forty-Five (545) Calendar days from the date of Notice To Proceed. You are required also to furnish proof of bonding/insurance within 10 calendar days of receipt of this letter. Please sign and return the enclosed copy of this letter indicating your acceptance of the same.
