DFAS Clauses VTC DFAS SEALS for VTC CONFERENCE ROOMS WALLS REQUEST FOR QUOTES (RFQ) NO: HQ0423-10-T-0009 This is a combined synopsis/solicitation (RFQ) for the purchase of twenty-three (23) DFAS Seals that will be wall mounted in all DFAS rooms supporting a video teleconference system in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This will be a Firm Fixed Price Contract type. NAICS code for this announcement is 339950 with small business size standard 500 employees. This acquisition is a total small business set-aside. SOLICITATION CLOSING DATE: All quotes must be received by Close of Business on date stated in this announcement. Preferred delivery method for submission of your quote is via email: carrie.fairbanks@dfas.mil. Please include at a minimum the following with your quote: • Unit price for 23 Vinyl print signs as described below with total • Contact Name, Phone Number and Email Address • Company CCR Cage Code or DUNS Number • Tax Identification Number • Business Size (Large, Small, etc) DESCRIPTION: Defense Finance and Accounting Service (DFAS) requires twenty-three (23) DFAS seals to be mounted by DFAS to the wall facing each VTC camera in all DFAS rooms supporting a video teleconference system. DFAS will provide several high resolution pictures to base reproduction of the seal within five (5) days from date of award. Each wall sign shall include the following specifications: • 36” x ½” thick acrylic piece with attached wall mount to hang onto the wall • The wall mount is to be a partial acrylic frame 21.5” long x 3” wide x 1/4” deep. The legs of the frame shall be 4” long x 3” wide x 1/4” deep. This partial frame shall be located on the back of the acrylic disk, centered relative to the seal and approximately 8” below the top of the disk. The inside of the frame shall be beveled to allow the frame to be hung on hooks or wall screws. • Vinyl print of the Defense Finance and Accounting Service printed on the front of the 36” x ½” acrylic wall sign • Each sign shall be shipped in a manner to protect from damage during shipment. Conformance to Specifications: You must supply the wall signs and associated items in accordance with the description as provided in this announcement. Unless an exception is specifically noted within your quote, your quote will be considered to be in complete conformance to the solicited requirements. Delivery: Installation is not required. All Items to be delivered FOB Destination to the addresses provided below within 30 days from receipt of high resolution pictures of the DFAS seal which will be distributed to the Contractor within 5 days of contract award. Prior to shipment the Contractor shall submit an image of the completed sign to the Government Point of Contact (GPOC) for review.
