- Request for Quotation 10-161-SOL-00010 Cover Letter - RFQ 10-161-SOL-00010 The Contractor shall provide all material, labor and equipment to replace the roof of the Dental Clinic at the San Carlos Indian Hospital. Specific work as follows: 1. Tear off existing roof system at all four roof scupper areas to remove existing scuppers. Enlarge scupper openings to accommodate new Duro-Last scuppers, or approved equal. Dispose of all existing roof system debris and other debris in an approved facility in accordance with all local, state and federal regulations. 2. Remove existing collector boxes, and replace with new Duro-Last collector boxes following the installation of the new Duro-Last Roofing system. 3. Install a Geotextile slipsheet over the existing roofing system. 4. Install a white, custom prefabricated, mechanically fastened 40-mil Duro-Last Roofing system, or approved equal, including prefabricated roof deck material and parapet material, and all accessories including prefabricated stacks, curbs and scuppers necessary to complete the installation. Parapet walls are to be encapsulated with Duro-Last parapet material. Install Duro-Last fascia bar and cover, or approved equal, to terminate the Roofing system. 5. Install 2-way breather vents every 1,000 square feet per Duro-Last specifications. 6. Provide all necessary permits, engineering, dump and disposal fees, and any other tests needed to meet state and local codes. 7. Provide a 15-year labor and material warranty with no exclusions for ponding water or consequential damages following the completion of the job and the final inspection.
