Description
SOLICITATION ADDENDUM STANDARD FORM - 1449 COVER LETTER This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 and FAR Part 13.3, as supplemented with additional information attached to this notice. The Social Security Administration (SSA) is seeking Medical Consultant Services set up in Blanket Purchase Agreements (BPAs). The Solicitation Number is SSA- RFQ-R04-10-1002. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-38. The associated North American Industrial Classification Code for Physicians Offices (except Mental Health) is 621111, with a small business standard of $10 million and for Physicians Offices (Mental Health) 62112, small business standard $10 million. This requirement is a 100% Small Business Set Aside. The total potential effective period of this BPA is 60 months from date of award. The Contractor shall not be required to accept any new orders beyond 60 months from the award date of this BPA. This acquisition is being conducted in accordance with the Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, and FAR Part 13, Simplified Acquisition Procedures. The selected offeror must comply with the following commercial item terms and conditions as well as any addenda to these clauses. The FAR clauses and provisions that apply to this acquisition are FAR 52.212-1, Instructions to Offerors - Commercial Items, FAR 52.212-4, Contract Terms and Conditions - Commercial Items. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items. In addition, offerors must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items with its proposal. Applicable terms and conditions, and representations and certifications are attached. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the requirements of this solicitation in terms of lowest price and delivery time. Responses to this combined synopsis/solicitation are due by Thursday, May 6, 2010. ALL ELECTRONIC QUOTATIONS (CD) SHALL BE CLEARLY MARKED WITH SOLICITATION NUMBER SSA-RFQ-R04-10-1002 ELECTRONIC QUOTATIONS (CD copies) MUST BE SENT TO: Social Security Administration OAG Division of Eastern Regional Contracts Attn: Lucrecia Applewhaite 61 Forsyth Street, S.W., Suite 22T64 Atlanta, GA 30303 FAXED RESPONSES WILL NOT BE ACCEPTED. All questions regarding this RFQ must be submitted in writing to Rickie Bowens, by April 28, 2010 at e - mail address : Rickie.Bowens@ssa.gov, NO LATER THAN 3:00 P.M., Eastern Time. The Government will make every attempt to answer questions which will not automatically result in the extension of the proposal due date. All questions and their respective answers will be made available to all prospective offerors in the form of solicitation amendments. NO PHONE CALLS WILL BE ACCEPTED. E-mail communications shall include the offeror's name, point of contact, e-mail address, phone, and fax numbers. Offerors must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov.
