Brand Name Letter This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442700740332; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-39, effective date 19 Mar 2010; Defense DCN 20100324, effective date 24 Mar 2010; and AFAC 2010-0402 effective date 02 Apr 2010. The North American Industry Classification System (NAICS) code is 339112. The business size standard is 100 employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 3841. Travis AFB intends to award a brand name purchase order for the following surgical requirement: CLIN 0001: Medtronic Bone Morphogenic Protein-Auto Graft Replacement Kits (large). Manufacturer Product Number - 7510800 Qty: 2 Each Award shall be made in the aggregate, all or none. The following provisions and/or clauses apply to this acquisition. Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors -- Commercial Items (June 2008).
