- The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for Part time Optometry services at the Rapid City PHS, IHS Eye Clinic in Rapid City, South Dakota. Contractor shall provide services an average of 2 days per week; Monday through Friday; approximately 100 hours to be fulfilled over the course of twelve months. The period of performance will be May 17, 2010 through May 16, 2011. The Contractor is to meet all technical criteria and perform according to the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-10-127, shall be in writing by email. Please provide the postal mailing address and email and indicate how you would like the RFQ package sent. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be April 13,2010. The closing date of the RFQ package is April 27, 2010.
