- Description
- The West Palm Beach Medical Center located at 7305 N Military Trail West Palm Beach, FL 33410, intends to issue a Request For Proposal (RFP) for Adult Day Care Services (ADHC) in the Stuart area. In accordance with the Statement of work and VHA Handbook 1147.1 dated 6 July 2009, the facility shall meet the latest edition of the Life Safety Code, the facility shall be licensed in the state of Florida or state approved, and the facility shall meet the standards of ADHC state licensing body. Period of performance shall be for (1) base year with (4) one-year options. Type of contract will be Firm Fixed Price, Basic Ordering Agreement. The Government reserves the right for a multiple award, if it is in the best interest of the Government. This procurement is being solicited as UNRESTRICTED; the applicable NAICS code is 624120. Solicitation will be electronically issued in commercial item format in accordance with FAR Part 12 and 15 on or about April 05, 2010 with an estimated offer due date of 05/05/2010. Any future amendments and other miscellaneous documents will be available electronically for download at the following URL: http://www.fedbizopps.gov. All responsible sources may submit an offer that if timely received, will be considered. Offerors are to be advised that an award may be made without discussion or any contact concerning the proposal received. However, the Government reserves the right to enter into discussion if deemed necessary, and if discussions are conducted, the Offerors within the competitive range will be afforded the opportunity to revise their proposal. Proposals are due on or about May 05, 2010 at 4:00 P.M. Eastern Time. Proposals are to be mailed in, the contractor must ensure that we receive the proposal on time confirming by e-mail. Telephone requests for the solicitation will not be accepted or honored. Offerors are responsible to submit all documentation requested and completely fill out the solicitation. Incomplete solicitation will not be considered for evaluation.
