- List of Attachments: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a quotation is being requested and a written solicitation will not be issued. Solicitation Number FA4417-10-T-0003 is hereby issued as a Request for Proposal. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-38, effective 10 December 2009. This acquisition is 100% set-aside for Small Business concerns. The North American Industry Classification System (NAICS) code for this project is 72221. The contractor shall provide all labor, equipment, materials, tools, parts, shipping, and all other items necessary to provide the following items: Smoothie/ Juice Bar and resale concessionaire services at the Hurlburt Aderholt Fitness Center as defined in the Statement of Work. This requirement will result in a concessionaire agreement between the awardee and the 1 SOFSS. This agreement is for a period of one base and four (4) one-year option periods. Delivery Terms are FOB Destination for delivery to 1SOFSS/FSFY Building 90304 Hurlburt Fld 32544-5267. The following provisions and clauses apply to this solicitation and will remain in full force in any resultant award: FAR 52.212-1, Instructions to Offerors - Commercial Items (June 2008). It is amended to read: Offeror shall submit signed and dated offer to 1 SOCONS/LGCA, Attn: A1C Tyler Davidson, 350 Tully Street, Building 90339, Hurlburt Field FL 32544-5810, by 26 March 2010 at or before 3:00 p.m. CST. Submit signed and dated quotations Schedule of Compensation marked with solicitation number FA4417-10-T-0003. The government reserves the right to evaluate proposals and award a contract without discussions with offerors. FAR 52.252-2, Clauses Incorporated by Reference (Feb 1998); FAR 52.252-6, Authorized Deviations in Clauses (Apr 1984); NAF GENERAL PROVISIONS (Attachment 1- 9.d.), Hurlburt Local Clauses HF 019, HF042, and HF043 (Attachment 1 - 9.e.). In the event that one (1) contractor wins this contract and the contract for Massage Therapy (Solicitation number FA4417-10-T-0002), the government holds the right to merge these contracts into one (1) contract, and management will be consolidated. Offers shall be evaluated in accordance with FAR 52.212-2, Evaluation of Commercial Items (Jan 1999) which is incorporated into this Request for Proposal, with an addendum to paragraph (a) as follows: The following factors shall be used to evaluate offers: (i)Price- Lowest price will be determined by the sum of all prices. (ii)In the event of similar lowest prices, the Government will evalute past performance. The Government will enter into an agreement with the technically capable and responsible offeror whose offer with the lowest price. The offeror shall submit with their offer; completed offer schedule, and certification documentation for employees forecasted to work on this requirement. FAR 52.212-3, Offeror representations and Certifications (Aug 2009) - Commercial Items, and the offeror must include a completed copy of this provision with their proposal or indicate (with DUNS Number) that information is available on the federal government website ORCA (https://orca.bpn.gov/).
