Description
Mobile Dental Exams April 2010 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is available on the FBO website. RFP Number W912PQ-10-T-0021 is being issued as a Request for Proposal. The solicitation documents and incorporated provisions are those in effect through Federal Acquisition Circular 05-26. This procurement is unrestricted. The cost magnitude is estimated to be $100,000 and $550,000. The NAICS code applicable to this RFP is 621210. The small business size standard is $7,000,000. Under the solicitation, the contractor is required to provide an on location mobile dental practice with the capability to provide examinations to approximately 740 New York Army National Guard Personnel. The place of performance is 2 locations. Place of performance for Event #1, Joint Forces Headquarters, 330 Old Niskayuna Road, Latham, NY 12110, and Event#2, Building T-855, Oswego Avenue, Fort Drum, NY 13602. The period of performance is Event #1, 9-11 April 2010, and Event#2, 30 April-2 May 2010.. All technical requirements pertaining to this solicitation are contained in the RFP Statement of Work. The provision at 52.212-1, Instructions to Offerors - Commercial applies to this solicitation. Award will be made to the lowest responsible offeror that is technically acceptable. The provision at 52.212-3, Offeror Representation and Certifications apply to this solicitation. Interested parties must complete the Representation and Certifications at the ORCA website. Contractors are encouraged to register as an interested vendor for this solicitation and check the FBO website frequently for any changes to the solicitation. Statement of Work: 1. BACKGROUND: Oral health is vital for force health protection. Failure to achieve a standard of oral health could preclude a Soldier from deploying. Dental Readiness is a command responsibility. DoD established a uniform dental classification in HA Policy 02-11. DoD policy requires dental screening examinations to be accomplished and documented on 90% of service members. Members of the Army National Guard shall receive an annual dental screening examination to determine their dental classification. The results of that screening examination shall be entered into the DENCLASS database no later than 1 day from the date of examination.. The DENCLASS database updates the Army Medical Protection System (MEDPROS). Dental entry must be completed within 2 days from date of Denclass data entry. 2. OBJECTIVES AND SCOPE: The objective of this Statement of Work is to provide parameters and criteria for the conduct and completion of the Deployment requirements for Dental for approximately 740 Non-Alerted Soldiers from multiple units of the 42 AVN, 102 MP's, 204 th ENG and 102 MP units utilizing a mobile dental facility (either RV style, hauled trailer or temporary building). The SOW will encompass 2 separate events at different locations and dates. The requirements include: • Clinical evaluation of the oral cavity by a New York licensed dentist employing visual clinical exam supported by bitewing dental radiographs. Panograph dental radiographs must represent the Soldier's current dental condition. Digital Panographs are to be taken if there is no panograph found in the record and in DENCLASS, or if the Soldier has had major dental treatment since the last panograph. Contract dentist may order a panograph if deemed clinically necessary to complete a proper dental exam. Radiographs will be taken by trained dental technicians. Panographs/bitewings that are digital images must have a printed photographic quality copy available in the dental health record and entered into DENCLASS by the contractor. • Each exam will be entered into the DENCLASS reporting system within 24 hours of examination by ASM Research (ASMR) trained personnel, annotated on the Standard Form (SF) 603 (Health Record - Dental) and on the DA 5570 (Health Questionnaire for Dental Treatment) of the Soldier's dental record. • Personnel performing the dental examinations have a moral and ethical obligation to inform the Soldier if they observe or are apprised of any sign or symptom for which the Soldier should obtain further evaluation or care. • A comprehensive dental examination identifies deficiencies. Dental treatments required to correct those deficiencies to deployment standards are authorized. When dentist is performing the examination and existing panoramic x-ray is on file, he/she must determine if significant changes have taken place where a new panoramic is required, i.e., multiple extractions or fillings. DENTAL EXAMS. Event #1: Perform a projected 400 dental examinations, to include digital bitewings; with digital panographic x-ray's. See Statement of Work. Place of Performance: JFHQ, Latham, NY Friday, 9 April 2010, 11:00 AM through Sunday 11 April 2010, 12:00 PM. Event #2: Perform a projected 340 dental examinations, to include digital bitewings; with digital panographic x-ray's. See Statement of Work. Place of Performance: Fort Drum, NY, Friday, 30 April, 2010, 11:00 AM through 2 May 2010, 12:00 PM. Contractor shall be licensed on any services rendered and equipment operated during mission. Radiograph equipment should be licensed under the State of New York. The Contractor shall ensure that all providers, technicians and other HCPs performing services under this agreement have current licenses, registrations, and/or certifications according to industry standard and as required for the specific state, commonwealth, district or territory in which they are providing contract services. The Contractor shall ensure that all HCPs have a current certification in basic life support.
