- Description
- The purpose of this modification is to incorporate the following changes: (1)Carpet replacement and installation is required in the rooms stated below. Dimensions are provided for reference purposes. Vendors are responsible for conducting their own measurements in the field: 1. Nursery (Room 207) - 15' x 17'1"2. Bedroom (Room 203) - 15'1" x 19'6"3. Corridor (Room 304) - 47" x 57" Carpet replacement and installation is not required in hallways or stairways. (2) The following paragraph replaces paragraph (ix) from the original solicitation: (ix) DOCUMENTS TO BE INCLUDED IN QUOTE (All documents shall be submitted via email, except the carpet sample): 1. Detailed specification including all descriptive and technical literature to be used during this project.2. A firm fixed price quote.3. A carpet sample to match the existing carpeting. Carpet samples shall be mailed to Gateway National Recreation Area, 210 New York Avenue, Staten Island, NY 10305, Attn: Cheryl Adamski, Contracting Office. 4. Applicable warranty information.5. Three (3) references of the proposed services within the past five years.6. Name, phone number, and address of your point of contact. (3) The following paragraph replaces paragraph (x) from the original solicitation: (x) The Government contemplates the award of a single purchase order on a "low-price-technically-acceptable" basis. A technically acceptable business will meet or exceed the following criteria: 1. Have at least five (5) years of experience performing similar services as required in this solicitation.2. Provide a carpet sample to match the existing carpet. (4) The following paragraph replaces paragraph B located in the Statement of Work from the original solicitation: B. SUBMITTALS: The contractor shall submit a CD containing all computer information used to formulate the weave and weave sequence for color and design to match the existing carpet, following contract award. The CD shall be submitted to the Contracting Office. (5) The due date and time for all submittals is extended to Wednesday, April 7, 2010 at 10:00am ET. All offers submitted via email must have a corresponding carpeting sample in order to be considered for evaluation. Offers shall me submitted via emailed to cheryl_adamski@nps.gov. Carpet samples shall be mailed to Gateway National Recreation Area, 210 New York Avenue, Staten Island, NY 10305, Attn: Cheryl Adamski, Contracting Office. (6) All other terms and conditions remain unchanged.
