Attachment 01 Schedule of Items Attachment 02 Offerors Representations and Certifications The Mt. Baker-Snoqualmie National Forest intends to award a contract for furnishing and delivery of High Density Polyethylene (HDPE) culverts with split couplers. Upon delivery of the culvert and signed certification, payment can be made for the purchase either by Government credit card or by Purchase Order. The final quantity of the culverts purchased may vary based on the prices quote and dollars available. As a Federal Agency, we do not pay State or Local sales tax. MATERIAL PROPERTIES: Virgin material for pipe shall be high density polyethylene conforming with the minimum requirements of cell classification 435400C (as described in the latest version of ASTM D3350, except that carbon black content should not exceed 5%). PIPE REQUIREMENTS: 24" diameter Pipe shall meet the requirements of AASHTO M294, Type S or ASTM F2306. DELIVERY LOCATION FOR ITEM 1: Pipe shall be in 20ft sections (see Attachment 01 Schedule of Items). The required delivery date is approximately end of March or early April 2010. Contractor shall contact the Skykomish Ranger District Office at (425) 677-2414 prior to delivery to the Skykomish Maintenance Compound. Pipe sections are to be delivered to: Skykomish Maintenance Compound located at 74920 N.E. Stevens Pass Highway, Skykomish, WA 98288 The contractor shall be responsible for the delivery of the pipe and the Government is responsible for offloading. CERTIFICATION: A signed certification shall be required upon delivery. INTERESTED VENDORS: Offerors are to provide pricing for Items as outlined in Attachment 01 Schedule of Items and along with a completed copy of Attachment 02 the provision 52.212-3 Offerors Representation and Certifications- Commercial Items with its offer RESPONSE DATE: All quotations received by March 25, 2010 by 4:30pm PST will be considered by the Government. Responses may be faxed to (425) 783-0129 or emailed to jhada@fs.fed.us. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. The solicitation number is AG-05M6-S-10-0027 and is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 58. The North American Classification System code is 326122 with a corresponding size standard of 500 employees. This project is a 100% set aside for small business concerns.
