- AMENDMENT 0004 THE PURPOSE OF THIS THE PURPOSE OF AMENDMENT 0004 IS AS FOLLOWS: a.Provide all potential Offerors with the attached Questions and Answers that better clarify Solicitation AG-3198-S-10-0003-Comb-Syn pertaining to SNAP Nationwide. b.Provide revised/changed page to SNAP Nationwide Store Visit Program RFP as summarized in the attached table and associated change page to clarify selected issues. c.Provide revised pricing Offeror Worksheet attached as SNAP_Store_Visit_Offeror_Pricing_Worksheet - Rev-2.xlsx. The revised worksheet allows the vendors to propose separate discount factors for (a) Excess of 25,000 store visits and (b) 30 day trips versus 10 day trips. d.Provide Attachment 2 of Enclosure 5, Past Performance Questionnaire as a separate file for ease of delivery to the Offeror's clients. e.The date for receipt of offers remains unchanged. All offers are due MARCH 29, 2010, 5:00PM, Eastern Daylight Time (EDT).
