- This modification is to add the RFP by an attached WORD document and also to clarify the following on the SF 1449: Block 12: This would be any discount terms your company would offer. An example would be a 2% percentage savings to the Government if we were to pay within 10 days of invoice versus 30 days from the invoice. You would leave this block blank if you were not offering a discount for prompt payment. Block 17a: This would be your companys name, address and phone number. Block 21: As stated in the solicitation on page 5, the Government estimates 50 record per year would be reviewed so you would enter "50" in this block. Block 22: Unit should be "Each" since this would be 50 records per year. Block 23: Enter the unit price cost per record review or for reading/diagnosing patient EEG/sleep and evoked potential records. Block 24: This would be the total amount. Block 22 times Block 23. You also need to complete the base year price and all four option years prices in the Schedule of Supplies/Services/Price/Costs section of the RFP (should be pages 5 and 6). The Quantity is 50 since this is the estimated amount of patient records that Wilmington VA Medical Center believes will be read/diagnosed. The Unit is entered as Each so you just need to enter the unit cost and total cost for the base year and the four option years. Blocks 30a, 30b, and 30c: These need to be completed and mailed to me in three copies by 25 Mar 10.
