Attach 5 - PP Questionnaire Attach 4 - Wage Det 1994-0761,Rev28 Attach 3 - Wage Det 2005-2511,Rev11 Attach 2, Estimated Quantities-Pricing Schedule (See NOTICE in Solicitation Description) Attach 1.1 CDRL Attach 1 - PWS The 49th Contracting Squadron is issuing this solicitation for a requirement for Packing, Containerization and Local Drayage of Direct Procurement Method (DPM) Personal Property Shipment Services at Holloman AFB, NM 88330-8277 for the remainder of calendar year 2010. The packing, containerization and local drayage contract provides a contractual arrangement for a contractor to furnish all personnel, equipment, facilities, supplies, services and materials for the preparation of personal property of Department of Defense personnel for shipment and/or storage and related services. Tasks include premove surveys, weighing, preparation, packing, loading and containerization, tagging, inventorying and making packing list, appliance servicing and unservicing, containers and vehicles, marking exterior shipping containers, storage, unpacking/unloading, preparing loss and damage reports, and providing documentation and reports. The work applies to Schedule I (Outbound Services), Schedule II (Inbound Services), and Schedule III (Intra-City and Intra-Area Moves) via domestic and international DPM for inbound and outbound household goods and unaccompanied baggage shipment and/or storage. The contractor provides services in a manner that meets or exceeds the standards in the Performance Work Statement (PWS) and only in connection with authorized Permanent Change of Station (PCS), Temporary Duty (TDY) or housing orders. All shipments for Holloman AFB, New Mexico will include: Area I – Otero County, Area II – Lincoln County, Area III – Chavez County, Area IV – Eddy County, and Area V – Lea County. All work shall be accomplished in accordance with the PWS based on Defense Transportation Regulation (DTR) 4500.9-R-Part II Cargo Movement, and DTR 4500.9-R-Part IV, Personal Property. Contractor is liable for full replacement value in the event of a claim for loss or damage. Work to be performed under the packing, containerization and local drayage contract will be within the North American Industry Classification System (NAICS) code 484210, Used Household and Office Goods Moving. This solicitation is a formal Request for Proposals (RFP) on Federal Business Opportunities (FedBizOpps). The solicitation will be result in a firm, fixed price contract with a base period of seven (7) months (1 Jun 10 – 31 Dec 10) and four (4) one-year option periods extending through 31 Dec 14. This acquisition is unrestricted. The successful contractor will be selected using performance/price tradeoff procedures in accordance with FAR 15.101-1, resulting in the best value for the Government. The entire solicitation is made available only on the website FedBizOpps www.fedbizopps.gov. No paper copies will be issued. Potential offerors are responsible for monitoring this site for any changes (amendments) to the release of this solicitation package. All amendments to this solicitation will be posted on this FedBizOpps site and all offerors must acknowledge receipt of the amendments when submitting their offers. In the evaluation of proposals, the 49th Contracting Squadron will make a best value decision where competing offerors' past and present performance history and technical capability will be evaluated on a basis significantly more important than price. To be eligible for contract award offerors must be registered in the DoD Central Contractor Registration (CCR). Register via the CCR website, www.ccr.gov. All responsible sources may submit a proposal that shall be considered by this Agency. Interested offerors are encouraged to submit their contact information by registering as an interested vendor utilizing the Vendor Notification Service link within the Register to Receive Notification function of FedBizOps. No bidders mailing list will be maintained. Interested offerors are also encouraged to submit their vendor information on the Online Representations and Certifications Application (ORCA), https//orca.bpn.gov. Offers will be submitted on the Standard Form 1449, Solicitation/Contract/Order for Commercial Items, and must include, as a minimum, the items listed in accordance with the solicitation.
