Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6 under Simplified Acquisition procedures in Subpart 13.5 as supplemental with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and an additional solicitation will not be posted. The solicitation number for this acquisition is VA-248-10-RP-0233 and is being issued as a Request for Proposals (RFP). The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-38. This acquisition will be processed under Simplified Acquisition Procedures. The associated North American Industry Classification System (NAICS) Code is 325120. The West Palm Beach VA Medical Center and the Miami VA Medical Center has a requirement for a vendor to supply medical grade bulk oxygen to each facility. This contract is seeking sources to provide all needed components to support a Food and Drug Administration (FDA) certified bulk oxygen main supply system for medical gas in accordance with National Fire Protection Agency (NFPA) 99. The contract will be based on rental of primary tanks for each VAMC location with a capacity of 6000 gallons and rental of a reserve tanks at each VAMC location with a 1500 gallon capacity. The contract shall include all required components of the bulk oxygen supply, connection to VAMC's existing oxygen system, preventive maintenance, annual training, inventory management of the bulk systems, and all necessary documentation. The supply systems will be located at the existing location at the VAMC in West Palm Beach, Florida and at the VAMC in Miami, Florida. The point of contact for the West Palm Beach facility will be Victor Cid-Guzman, COTR and the point of contact for the Miami facility will be Ryan Jackson, COTR. The systems at both facilities shall be maintained to ensure uninterrupted supply of oxygen for both multi-million dollar medical centers.PERIOD OF PERFORMANCE: Base Performance Period: June 1, 2010 - December 31, 2010 1st Option Year Period: January 1, 2011 - December 31, 2011 2nd Option Year Period: January 1, 2012 - December 31, 2012 3rd Option Year Period: January 1, 2013 - December 31, 2013 4th Option Year Period: January 1, 2014 - December 31, 2014 CONTRACTOR ELIGIBILITY REQUIREMENTS: The contractor' business must be physically located within a 50 mile radius of each medical center. All work shall be done in accordance with NFPA, Center for Disease Control (CDC), The Joint Commission and American Society of Heating, Refrigerating and Air-Conditioning Engineers (ASHRE) Guidelines. o Vendor must be FDA certified to produce medical grade liquid oxygen daily needs as well as in the event of an emergency situation, i.e. hurricanes, flooding, tornadoes, etc. - Documentation is required.o Must be able to monitor via telemetry monitoring system to manage levels of medical gas. o Vendor must be certified to perform and complete FDA annual inspection for bulk liquid oxygen system. o Vendor must have trained and certified staff to perform any required maintenance and inspections at each local site or available within 2 hours of time of notification. o Vendor must be able to deliver Monday-Friday (with the exception federal or training holidays) between the hours of 0800 and 1630 for the West Palm Beach VAMC and between the hours of 0700 and 1530 for the Miami VAMC. Vendor must be available to make emergency deliveries within 1 hour of notification as required or as mission dictates. o Vendor must provide packing list at time of installation/delivery to ensure proper payment. o Vendor must make all records available ON-LINE (web access) including delivery history, level history, reading history, and account information that is downloadable in spreadsheet format. o Must have an emergency contact system in place in the event of catastrophic or emergency situations. CONTRACTOR PERFORMANCE REQUIREMENTS: Vendor Shall Supply contractor owned: All delivery, installation, labor, materials and pick up charges to be paid by the vendor. o The shall maintain uninterrupted service to the hospital in the events of a failure of the primary tank or until the primary tank can be refilled (monthly usage estimated to be 466,000 SCF at the West Palm Beach VAMC and 393,000 SCF at the Miami VAMC). o A reinforced concrete pad which meets the minimum standards in the existing locations o A Pressure Control Manifold, all lines/piping, connections, required to connect tanks to manifold and to existing hospital piping at pad location. o A Pressure Control Manifold, all lines/piping, connections, required to connect tanks to manifold and to existing hospital piping at pad location for reserve. o An off-site monitoring system (telemetry system), in which the successful vendor will be able to independently monitor the system and refill requirements. o Provide all equipment required to fill tanks as needed. o Provide Purity of Oxygen: 99.996% - Medical Grade (a.k.a. USP Grade) o Frequency of Fill: As Needed o Vendor shall own and maintain the tanks, manifold system, and all associated equipment from the tanks to the connection point at pad site. o Contractor must provide emergency service plan documenting what services will be available in the event of complete loss of system, catastrophic event or emergency situations. o At the end of the contract the contractor is responsible for removal of all tanks and equipment at no cost to the Government. o HOURS OF DELIVERY: Monday - Friday 0800 to 1630 for West Palm Beach VAMC and Monday - Friday 0700 to 1530 for Miami VAMC as well as available to either location within 1 hour of notification for emergency situations 24/7. ROUTINE SERVICE: The contractor will provide routine service Monday through Friday, 0800 to 1630 at the West Palm Beach VAMC and between the hours of 0700 to 1530 at the Miami VAMC. On-site service will be provided within (2) hours, excluding holidays, upon notification. PREVENTATIVE MAINTENANCE: The contractor shall provide preventive maintenance, emergency response, routine service, and inventory management for the Bulk oxygen system as required by NFPA 99, JCAHO, VA and other related codes/standards. o Provide a complete written preventive maintenance report to the project official listing operation of each device as required by referenced codes. o Provide documented annually operational training on site to associated personnel. During the scheduled inspections as required by referenced codes, the contractor will perform a thorough maintenance check of the system including but not limited to: o Document training for safety plan and daily operations. o Document Gas emergency plan JCAHO EC.1.4 o Reserve supply change over system o Vaporizers and automatic switch assemblies o Document and inspect all tank and control valves. o Check and document all main source alarms for both tanks. o Check and document all gauges o Check and document operation of all pressures regulators. o Inspect and remove icing from vaporizer as required. o Provide a general inspection of all system components including wiring, contacts, air pressure switches, belt tension etc. Daily inspections of tank site and tank levels will be performed by assigned VA staff. Maintenance of site grounds is the responsibility of each VA facility. EQUIPMENT REPAIR not listed above will need prior approval from Contracting Officer before the execution of any additional work. PAYMENT SCHEDULE: This contract is billable on a monthly basis. All requirements of this contract must be satisfied before the monthly payment is approved. REQUEST FOR QUOTATION BULK OXYGEN SERVICE: Please submit a qualifications package addressing past performance and technical ability along with a quotation for the following services: Pricing Page: Based on average monthly usage previously noted for each location. MAIN SUPPLY SYSTEM TANK, Product Price, Fuel Surcharge, Energy Surcharge, Hazmat, Certificate of Analysis, Tank Rental for West Palm Beach VAMC (12 months @ $_______ for primary tank with 6000 gallon capacity and 12 months @ $________ for reserve tank with 1500 gallon capacity) as well as Tank Rental for Miami VAMC (12 months @ $_______ for primary tank with 6000 gallon capacity and 12 months @ $________ for reserve tank with 1500 gallon capacity). ADDITIONAL REQUIREMENTS: "15 calendar-day advanced notice is required before change over operations can begin. "Contractor shall coordinate all work, inspections, and repairs with the COTR for each facility. "Delivery records, receipt and certification records will be maintained by the COTR at each facility. "Specific delivery locations for each facility will be designated by the COTR. "Contractor shall provide all necessary labor, materials, tools, equipment, and supervision to execute this contract. VA will provide NO tools, materials, equipment, and labor. "Contractor shall provide a schedule of contractor provided maintenance requirements and related documentation when completed. "Contractor shall provide two - 4 hour training classes, and related training materials for VA staff as scheduled by VA. "All work shall be in accordance with NFPA, NEC, OSHA, FDA, VA and other related codes and standards. "Vendor is required to check in with VA Police after hours immediately upon arrival on VAMC property whether at West Palm Beach VAMC or at Miami VAMC. This contract shall be for a base period of 6 months with option to renew an additional 4 one-year periods. Parties responding to this solicitation may submit their offer in accordance with their standard commercial practices (e.g. on company letterhead, formal quote form, etc.) but must include the following information: 1) company's complete mailing and remittance addresses 2) discounts for prompt payment if applicable; 3) Dun & Bradstreet number; 4) Taxpayer ID number; 5) Catalog or Published Price Listing applicable to the service; 6) NOTE: Offerors must meet specifications as noted in the synopsis. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications/Commercial Items with their offer. Note: Contractor must be registered with the Central Contractor Registration (www.ccr.gov). The Government intends to make a single award based on the best value to the Government: evaluating price, past performance and technical ability. The Government reserves the right to make an award without any further discussion. Interested offerors may submit proposals to include pricing data, as well as supportive documentation related to past performance on jobs of similar size and scope, as well as technical ability to perform work as required, by mailing proposals to: West Palm Beach VA Medical Center, John Hull, Contracting Officer, 7305 North Miliary Trail - Building 12 (548/90C), West Palm Beach, FL 33410 only within 15 days from the posting of this notice no later than 03/16/2010 at 3:00 p.m. EST.
