- Services, Personal -Provide One Oral Maxillofacial Surgeon to examine, diagnose, and treat by surgical means abnormalities of oral and maxillofacial structures at Keesler Air Force Base Medical Center. Services will include the surgical removal of diseased, impacted and malposed teeth, biopsies, alvelolectomies, consultations and follow-up of systemic conditions that have oral involvement. This requirement is UNRESTRICTED with a NAICS code of 621210 and a small business size of $7 million. The period of performance is 1 May 10-30 Sep 10, plus 4 one-year option periods. All responsible sources may submit a quote and at least 2 references, which will be considered by this agency. This request for quote is issued under Simplified Acquisition Procedures (SAP) and award will be made in the form of a firm-fixed priced contract. The solicitation will be posted to http:www.fedbizopps.gov/ on or about 8 Mar 2010. The solicitation may be downloaded for FedBizOpps, since copies will not be available for mailing. All prospective offerors must be registered in the Central Contractor Registry (CCR) prior to award of contract with any DOD agency. Registration maybe accomplished at http://www.ccr.gov.
