- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. SYNOPSIS 1.Product and Performance Description. The Oklahoma Army National Guard expects to enter into a blanket purchase agreement for Occupational Medical Surveillance Examinations for full-time support and/or temporary personnel in Norman and Tulsa, Oklahoma. This is an unrestricted procurement. No DPAS rating is applicable. 2.Quantities Government expects to issue a blanket purchase agreement followed by one or more delivery orders for services to be performed on a firm fixed price basis to the Contractor whose proposal represents the best value to the government. 3.Term Task orders shall be issued for a period of one year following issuance of the blanket purchase agreement. Services pursuant to each task order shall be performed at such time as is designated in the task order, not to exceed the end of the fiscal year in which the task order is issued. 4.Payment Payment may be made by either of the following methods, at the option of the Government: (1) Government Purchase Card or (2) by EFT or credit card with Net 30 payment terms following completion of services and receipt of the invoice. 5.Delivery Deliveries shall be FOB Destination to the Governments location. 6.Inspection and Acceptance Inspection and Acceptance shall be by the Government, within seven (7) days of delivery, and shall occur at the delivery location. 7.Deadline for Questions and Proposals All questions regarding this Solicitation must be submitted on or before 5:00PM CST March 12, 2010, IN WRITING (to include fax or e-mail) to Mrs. Sarah Lane at 405-606-7983 or sarah.lane@us.army.mil. Questions will be posted together with answers in the form of an amendment to the Solicitation on or around 5:00 PM CST March 16, 2010. Proposals are due by noon CST, 26 March, 2010 by mail or personal delivery. 8. Availability of Updates Via Internet The combined synopsis and solicitation can be downloaded and printed from the World Wide Web at https://acquisition.army.mil or for additional information e-mail Mrs. Sarah Lane, sarah.lane@us.army.mil. Potential Offerors may register for automatic updates, and it is incumbent upon the interested parties to review the web site frequently for updates to the documents.
