- JUSTIFICATION AND APPROVAL (J&A)FOR OTHER THAN FULL AND OPEN COMPETITION VA Palo Alto Health Care System 3401 Miranda Ave. Palo Alto, CA 94304 Subject: VA-261-10-RQ-0113/VA Palo Alto Underground Storage Diesel Fuel Tank Sensors Repair CONTRACTING AGENCY: Department of Veterans Affairs VISN 21 Consolidated Contracting Activity (CCA). 2. DESCRIPTION OF ACTION: Facilities Engineering at VA Palo Alto has an urgent requirement for repair of leak sensors in two underground storage tanks at VA Palo Alto. This procurement will be accomplished by the Consolidated Contracting Activity (CCA90), VISN 21, located at the Palo Alto facility. 3. DESCRIPTION OF SUPPLIES/SERVICES: Summary of Requirements: Vendor to provide aforementioned services as described in #2 above. Estimated value of requirement, one time: $35,000.00 4. AUTHORITY CITED: FAR 6.302-1 5. REASON FOR AUTHORITY CITED: The above cited authority provides for purchase from a sole source whenever that source is the only responsible source and no other supplies or services will satisfy agency requirements. David Shiraishi, GEM Coordinator and VA Palo Alto Environmental Program Manager, advises the intended awardee, TEC Accutite, is the only EPA/State of California/County of Santa Clara designated/registered contractor available to furnish the service, in accordance with Clean Water Act requirements. 6. EFFORTS TO OBTAIN COMPETITION: Competition not available per #5 above. 7. FAIR AND REASONABLE PRICE DETERMINATION: A Price Negotiation Memorandum will be prepared which will detail the actions taken by the Contracting Officer to ensure fair and reasonable pricing. INTERESTED SOURCES: Award to be made to TET Accutite of South San Francisco, CA. 11. AGENCY ACTIONS TO INCREASE COMPETITION: Per #5, TEC Accutite is the only available firm for this effort. 12.
