Description: SynopsisThis is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-38. The North American Classification System (NAICS) number is 485310 and the small business size standard is $7 million average annual receipts for the past three years. This procurement is set-aside for small business. Offerors are encouraged to submit proposals based on the work requirements specified herein for a Base Year from April 1, 2010 or date of award through March 31, 2011, with two (2) option yearsOption Year-1 from April 1, 2011 through March 31, 2012 Option Year-2 from April 1, 2012 through March 31, 2013. Trips are Estimated Annual Quantities and Actual Quantities May Vary. This contract will be awarded in Total Aggregate. SECTION B-DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK B.1.Purpose: The Central Arkansas Veterans Healthcare System (CAVHS), 4300 West 7th Street, Little Rock, Arkansas and 2200 Fort Roots Drive, North Little Rock, Arkansas has a requirement for twenty-four (24) hour transportation services for the beneficiaries (patients) and packages. This is a requirement that is needed for one-way and round-trip transports for various locations in and around the city for a base period plus two (2) one (1)-year options. B.2. GENERAL DESCRIPTION: The Contractor shall furnish all necessary personnel, equipment, insurance/certification and vehicle required to provide transportation services for the beneficiaries (patients) and packages of the Central Arkansas Veterans Healthcare System (CAVHS), 4300 West 7th Street, Little Rock, Arkansas and 2200 Fort Roots Drive, North Little Rock, Arkansas in accordance with all terms, conditions, provisions, and clauses of this solicitation. B.2.1. Group I: Transportation of the beneficiaries (patients) between CAVHS, 4300 West 7th Street, Little Rock, Arkansas and 2200 Fort Roots Drive, North Little Rock, Arkansas. A Group I Item trip shall be defined as a round trip from the single point of origin to the point of destination and back to the point of origin. Each trip will have a maximum of four (4) beneficiaries (patients). The Contractor shall pick up beneficiaries (patients) at the location specified by the Contracting Officers Technical Representative (COTR) or designee and the COTR or designee shall direct return trips. A Group II Item trip shall be defined as a round trip from the single point of origin to the point of destination and back to the point of origin. Each trip will have a maximum of four (4) beneficiaries (patients). The Contractor shall pick up beneficiaries (patients) at the location specified by the COTR or designee and the COTR or designee shall direct return trips. B.2.3. Group III: Notification will be given to the Contractor upon request of service to transport mental health beneficiaries (patients). The driver shall pickup beneficiaries (patients) as directed by the COTR or designee and must accompany beneficiaries (patients) from inside the point of origin. The drivers shall wait at the destination point for the beneficiaries (patients) and accompany the beneficiaries (patients) from inside destination point back to the Eugene Towbin VA Medical Center, 2200 Fort Roots Drive, North Little Rock. The driver shall accompany the beneficiaries (patients) at all times until inside the destination point or returned to an authorized individual at the Eugene Towbin VA Medical Center, 2200 Fort Roots Drive, North Little Rock.. In no instance will Contractor drop beneficiaries (patients) in the hall or entrance of the Eugene Towbin VA Medical Center, 2200 Fort Roots Drive, North Little Rock.. The driver shall be limited moving a maximum of one (1) beneficiary (patient) and, if applicable, additional staff member on each trip. These specialized transportation services are subject to include the schedule rates. B.2.4. Group IV: The Contractor is required to transport as many packages as can reasonably be carried by an average individual. A Group IV trip shall be defined as a one-way trip from the single point of origin to the point of destination. The Contractor shall deliver each package to the specified building and room number provided. Package deliveries shall be one-way. SPECIAL TRIPS: The Contractor shall provide a driver used in the performance of this contract to pick up daily Lab specimens. Specimens from the Lab, Bldg. 66 2nd Floor, NLR/Lab, Room 2D154, LR within 30 minutes of notification and deliver to the destination within thirty (30) minutes. (See b. c. and d. for daily pick-up of Lab specimens. The driver shall not leave specimens at the Lab without approval from CAVHS Lab personnel. B.2.5. Group V: Miscellaneous trips are not otherwise identified a per mile rate (one-way) shall be charged per schedule rate. Each trip shall be one-way and will include a maximum of four (4) beneficiaries (patients). B.3. GENERAL REQUIREMENTS: B.3.1. Orders: Prior to award of contract, contractor must provide evidence of the following to the CAVHS: a. Number of vehicles available to support the VA requirements b.Type of operating vehicles c.Condition of vehicles d.Number of staff e.Staff experience and qualifications B.3.2. Requests for services to transport beneficiaries (patients) and packages will be made in writing, by telephone, or by FAX. Only the Transportation Clerks or Administrative Officer of the Day (AOD) will make orders, requests and charges to transport beneficiaries (patients) described in Group I, II, III, and V. A list of specific individuals authorized to request services will be provided after award identifying which contract items that individual is authorized to request service for. B.3.3. Charges will also be provided in writing by the Contracting Officer. This notification will identify which contract items these individuals are authorized to request service. B.3.4. There are instances that the Contractor may be required to furnish up to 16 vehicles. B.4.Central Arkansas Veterans Healthcare System Waiting Time: B.4.1. The Contractor shall be on time for its scheduled pick-ups as weather permits. The CAVHS beneficiaries (patients) and packages shall be picked up within thirty (30) minutes of scheduled pick-up time. This includes pick-ups at any location (i.e. labs, clinics, ward areas, private residences, etc.). B.4.2. In no instance is a beneficiary (patient), package and lab specimen to wait longer than thirty (30) minutes after scheduled pick-up time or receipt of a pick-up order for an unscheduled pick-up. In case of no show the driver will immediately contact the COTR or designee and advise. B.5.Drivers: B.5.1. The Contractor shall maintain a record of each employee as to the character, current driving record and physical capabilities of performing the duties of a vehicle driver. The Contractor will make these records available for inspection upon request by the Contracting Officer, COTR or designee. B.5.2. Drivers will not be allowed to transport the CAVHS beneficiaries (patients) if they appear to be ill or under the influence of drugs or alcohol. Divers will display professional manners at all times. Rude or obscene behavior or language will not be accepted. B.5.3. Drivers shall wear appropriate attire and provide a city permit that clearly identifies the company they work for at the time of the pick-up. B.5.4. Drivers shall advise all beneficiaries (patients) and passengers to use the security devices (safety belts) prior to transport. B.6. Number of Beneficiaries: A maximum of four (4) authorized passengers; excluding Group III. The COTR or designee will notify the Contractor in advance when non-beneficiaries are authorized to transport. The COTR may in the best interest of the beneficiary (patient), allow one relative or one caregiver to accompany him/her. A relative is defined as a spouse, parent, son, daughter, brother, sister, uncle, aunt, niece or nephew of the beneficiary by blood or marriage. B.7. Rates: B.7.1. For all one-way trips under the contract, the Contractor shall receive the flat rate awarded for any trip. B.7.2. Return trips shall be directed by the COTR or designee and shall be billed at the same price as the trip out (Group I and Group II). The Contractor shall also receive the mileage charge awarded for any one-way trips in excess of the flat rate throughout the State of Arkansas (Group V). B.7.3. Each trip shall be one-way, excluding Group III Items and will include maximum of four (4) beneficiaries (additional surcharge per passenger will be included in the fare). B.7.4. The Contractor shall also receive the flat rate for no show beneficiaries (patients) awarded for any trip. B.8. Toll Charges: It is agreed and understood that the process quoted in the schedule do not include any ferry, bridge, tunnel or toll charges. Any such legitimate toll charges incurred shall be limited to two-way only and shall be listed separately on Contractors trip tickets. B.9. Vehicles/Equipment: B.9.1. The vehicle fleet and its equipment must be of the quality, model, and condition to assure the beneficiary (patient) high quality transportation and shall meet the minimum standards for maintenance, equipment, and staffing of all vehicles operated. Vehicles must have adequate straps, seat belts, seats necessary to secure beneficiaries (patients) and baggage. Contractor must have a record of preventative maintenance of all vehicles being used for service to CAVHS. Vehicles and maintenance records will be made available to the CAVHS for bi-annual inspections or whenever the Eugene Towbin VA Medical Center feels such inspections are needed. B.9.2. The vehicle used in fulfilling the terms of this contract shall meet the minimum vehicle requirements as mandated by the State of Arkansas. B.9.3. Beneficiaries (patients) and Contractor are prohibited from smoking in vehicles. The smell and residual smoke may cause other beneficiaries (patients) who will be riding in the vehicle to become ill. B.10. Evidence of Coverage: Before award of the contract, the Contractor shall furnish to the Contracting Officer a Certificate of Insurance (See FAR Clause 52.228-10) containing an endorsement to the effect that cancellation or any material changes in the policy (ies) which adversely affect the interest of the Government shall not be effective unless a thirty (30) day written notice of cancellation or change is furnished to the Contracting Officer. The term will endeavor is not acceptable as issued by Insurance Carrier. B.11. Representative of the Contracting Officer: After award of a contract, the Contracting Officer will designate an authorized representative of the Eugene Towbin VA Medical Center as the Contracting Officers Technical Representative (COTR), to order services (transport beneficiaries (patients) and packages) and to accept or reject services performed as to quality and condition. A copy of this designation will be furnished to the Contractor and COTR or designee will be primary point of contact for the Contractor at the CAVHS. Upon award, the Contractor will provide a primary point of contact for the COTR or designee to coordinate the daily operational issues. The COTR or designee will periodically make site visits to each area of operation in order to gain and maintain a familiarity with internal operations. B.12. Payments: B.12.1. Payment will be made Net 30 days in arrears upon receipt of the proper bi-weekly invoice. An itemized invoice with trip ticket summary must be submitted bi-weekly for the trips approved. Summary total charge for each trip that identify which category (Group) is applicable. Summary shall not reflect the beneficiaries (patients) name/or personal identification. B.12.2. Trip tickets will be provided bi-weekly to the CAVHS designated receipted site. Trip tickets are due no later than the first work day after the transportation was provided. Trip tickets must indicate date of trip, one-way or two-way rate, number of miles (if billing for mileage), origin and destination and any other charges by category (Groups) and total cost of trip. B.12.3. Invoices will be reviewed and reconciled to the trip tickets and travel logs. Unauthorized charges will be suspended and returned to the Contractor for clarifications.
