Description
This solicitation is 100% SET ASIDE FOR SMALL BUSINESS for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12, and Veterans Administration Acquisition Regulations as supplemented with additional information included in this notice. Solicitation number RFQ VA-248-10-RQ-0202 applies and is issued as a request for quotations. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-38. The North American Industry Classification Systems (NAICS) code is 541380, size standard $12.0 million. The James A. Haley Veterans Hospital (JAHVH) is soliciting quotations from qualified, responsible firms who are capable of providing all labor, equipment, transportation and parts necessary to provide Radiation Protection Survey Services at the James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd., Tampa, Florida 33612 in accordance with the Schedule of Services and Prices for the period of April 1, 2010 through March 31, 2011. Contract for Radiation Protection Survey requirements of JCAH Standard i.e., (I) Policies/Procedures Review, (II) PT Dose Measurements (A) and (B), (III) Equipment performance and (IV) Quality Control. Must be a qualified radiations physicist which is defined as an individual who is certified by the American Board of Radiology (ABR) in the appropriate disciplines of radiological physics including diagnostic, therapeutic and medical nuclear physics or who demonstrates equivalent competency in the disciplines. Offerors must be a legally registered business in the state of Florida. Period of contract shall be from April 1, 2010 through March 31, 2011 with four optional years. This solicitation will be made available only on the Federal Business Opportunities website http://www.fedbizopps.gov/ with an anticipated closing date of March 8, 2010. No paper copies will be issued. Offerors are responsible for obtaining any and all amendment or additional information via the fedbizopps website. Potential contractors must be registered in the Central Contractor Registry to be eligible for award (see Internet site: http://www.ccr.gov/).. All quotations are to be submitted by either email, sandra.jackson@va.gov, FAX (813) 979-3637, to the attention of Sandra T. Jackson (PBD) or hard copy to the designated office on or about COB March 4, 2010.
