- Amendment 0001 issued for including contract drawings. The purpose of this Amendment 0001 is to incorporate the attached drawings. The offeror is required to acknowledge receipt of this Amendment by signing in block 15 of this Standard Form 30. In addition, Section I.I. of the Agreement is deleted in its entirety and replaced with the following: I.I Statement of Work, specifications, Drawings, Exhibits, and Other Attachments 1. Wage Determination General Decision Number MN080075 Dated 12/04/2009 4. Offeror Representations and Certifications Form CIC221 Dated XXXXXX (provided by Contractor) 5. Small Business Subcontracting Plan, Dated XXXXXXXX (provided by Contractor) Proposal remain due as listed in Section I.E. Receipt of Offers in the Solicitation. All other terms and conditions not noted in this Amendment 0001 remain unchanged and in full force and effect.
