Contractor Questionnaire Bid Schedule SCA Wage Determination Solicitation Provisions and Contract Clauses SF-1449 REQUEST FOR PROPOSALS The United States Department of Agriculture, Natural Resources Conservation Service seeks proposals from contractors qualified to complete restoration of native vegetation at a 166 acre floodplain easement at Minto-Brown Island Park located in Salem, Oregon. The floodplain easement is comprised of four parcels (fields). The western parcel (Field 4) is separated from the two eastern parcels (Fields 1&3) by a large slough. Fields 1, 3, and 4 were active cropland until the final harvest in the fall of 2009. Field 2 is an old cherry orchard that has seen little or no maintenance for a number of years, but is not included in this scope of work. Within the project, USDA has identified six native plant communities to be restored. Each community requires a distinct set of treatments to complete the restoration. These treatments are outlined in the bid schedule and are described in detail in the specifications. Differing treatments including weed and non-native species control and removal, site preparation, orchard removal, mowing, planting, and seeding are to be administered to the four parcels that comprise the easement over a four-year period as shown in the accompanying USDA Bid Schedule, Construction Drawings, and Specifications. The Government intends to make award of a single firm, fixed-price contract without discussions. I. How to Submit a Proposal: (a) Complete the Bid Schedule: Contractors interested in submitting a proposal shall complete the Bid Schedule by indicating its proposed prices for all items in the Bid Schedule. (b) Complete Contractor Questionnaire: Contractors shall complete the questionnaire showing up to seven projects that the Contractor chooses to best describe their qualifications for this project. (c) Complete Contractor Disclosure Form: The Contractor shall complete the disclosure form indicating all first-tier subcontractors and suppliers. II. Minimum Criteria: To be considered, proposals must show that the offeror (a) has liability insurance for the activities described in this solicitation, (b) is registered in the Central Contractor Registry (www.ccr.gov), and (c) demonstrates that it has at least three projects within the past seven years in planting and maintaining native trees, shrubs, and grasses in the Willamette Valley, Oregon. III. Evaluation Factors for Award: Offerors whose proposals include a completed Bid Schedule and show they satisfy the Minimum Criteria, will be evaluated 50% based upon total price shown in the Bid Schedule, and 50% based upon past performance as indicated in the Contractor Questionnaire. The questionnaire must be completed with a minimum of three projects of 30 acres or more that include Floodplain, Wetland or Oregon White Oak plantings and weed control. Contractors must list client references so that past performance completing contract work on time and within budget may be assessed. IV. Proposal Due Date: Proposals are due at 4:00 pm local time on 8 March 2010. Optional Pre-Proposal Site Showing: A pre-proposal site showing will be held on February 25, 2010 at 1:00 PM at Minto-Brown Park North Parking Lot. VI.
