- This is a combined synopsis/solicitation for commercial supplies and services prepared in accordance with the format in Subpart 12.6 and Far Part 13 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This requirement is 100% set aside for small businesses. The solicitation is being issued as Request for Quote #VA-246-10-RQ-0144. The solicitation document and incorporated provisions and clauses are those in effect through the most recent Federal Acquisition Circular. The NAICS Code is 423450. The Department of Veterans Affairs Medical Center, Durham, North Carolina has an urgent requirement for a Perimed Periflux system laser doppler and intends to solicit quotes and award a firm fixed price contract. The contractor shall provide the following: (1) Diode laser 780nm wavelength PF5001 main unit including calibration device - 1 each (2) Laser doppler perfusion model PF5010- 4 each (3) Small angled thermostatic probe PR457-2 each (4) Small angled thermostatic probe kit to include PF105-3 adhesive strips-2 each (5) TC main unit including start accessory kit PF5002-1 each (6) Temperature unit PF5020-2 each (7) Transcutaneous module PF5040-2 each (8) Transcutaneous TCP02 electrode E5250-2 each (9) Perisoft for windows examination manager basic including disk, users manual 2 PF490 cables and 2 USB com-1 serial adapters-1 each. The following provisions apply to this solicitation: 52.211-6 Brand Name or Equal, 52.212-1, Instructions to Offerors-Commercial, 52.212-4, Contract Terms and Conditions-Commercial, Commercial Advertising, 852.271-70, 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial. Bidders are to submit pricing. Technical information is required if the item quoted is not Perimed. Process of determining contract award: award will be to the responsive, responsible bidder who can provide the Perimed product or a product that meets or exceeds the Perimed laser doppler specifications. Information on an alternative product should be specific, concise, and complete and provide sufficient information to demonstrate the product's capacity to meet or exceed the requirements. The information required in provisions 52.212-1, 52.212-2 and a completed copy of provisions 52.212-3 Offeror Representations and Certifications are to be provided with the quote. Bidders desiring to obtain copies of the reference provisions may contact this office or access clauses electronically at http://www.arnet.gov/far/or http://www.va.gov/oa&mm/vaar. Quotes, including price and technical data, must be received by February 5, 2010 at noon local time. Due to the urgent nature of this request, the offeror shall guarantee delivery of the item not later than February 29, 2010. Proposals shall be submitted to barbara.simmons@va.gov.
