- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-10-T-0039. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-38. The North American Industry Classification System Code (NAICS) for this procurement is: 621511; the Standard Industrial Classification Code is 8071. This requirement is (UNRESTRICTED). Eisenhower Army Medical Center, Fort Gordon, GA has a requirement for the following: Item No. 0001: Establish a Blanket Purchase Agreement to provide all anatomic and Hematopathologic Laboratory services. The hematopathology services required is 48- hours turnaround time of flow cytometry results including scatter plots which are available to the pathologists through an online and faxed method. The report results should have the capabilities of being entered into the DDEAMC Laboratory Information System by a cut and paste method reducing another potential for transcription error. All services will be provided to Dwight D. Eisenhower Army Medical Center Department of Pathology five (5) days per week during normal duty hours 0730-1630 hours excluding legal public. The required supplies and instruments shall be manufactured by US Lab Inc. Payment will be made by 25K Purchase Card. The period of performance will be from 08 February 2010 through 30 September 2014. THE SOLICITATION MUST BE QUOTED ALL OR NONE. To be delivered to: Dwight D. Eisenhower Army Medical Center, ATT: Pathology Department, Bldg 300 E Hospital Road, Fort Gordon, GA 30905.
