- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-10-T-0029. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-35. The North American Industry Classification System Code (NAICS) for this procurement is: 339112; the Standard Industrial Classification Code is 3841; and the small business size standard is 500. This requirement is (UNRESTRICTED). Blanchfield Army Community Hospital, Fort Campbell, KY has a requirement for the following: Item No. 0001: Establish a Blanket Purchase Agreement to provide all labor, material, equipment and transportation necessary to supply various medical operating room supplies necessary to perform various surgical and or orthopedic procedures (includes a wide variety of orthopedic surgical instruments, screws, plates, etc.) in the Operating Room, Department of Surgery, for Blanchfield Army Community Hospital, Fort Campbell, KY. The required supplies and instruments shall be manufactured by Synthes USA Sales LLC. Payment will be made by 25K Purchase Card. The period of performance will be from 01 February 2010 through 30 September 2014. THE SOLICITATION MUST BE QUOTED ALL OR NONE. To be delivered to: Blanchfield Army Community Hospital, ATTN: Operating Room, Bldg 650 Joel Drive, Fort Campbell, KY 42223-5349.
