- The following is a combined synopsis/solicitation: Eielson AFB, Alaska is issuing a requirement for a Film Digitizer. This announcement constitutes the only solicitation; proposals are being requested, and a formal written solicitation will not be issued. Solicitation number FM500492680078 is hereby issued as a Request for Quote (RFQ) on 07 January 2010. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-23, 25 January 2008. This acquisition is 100% set aside for small business concerns and is under North American Industry Classification Standards (NAICS) code 423450 with number of employees not to exceed 100. DESCRIPTION OF REQUIREMENT: CLIN 0001: Film Digitizer. Vidar Diagnostic Pro Advantage Film Digitizer model number 16323/13042 or equal.-1 each CLIN 0002 DIACOM Modality Worklist Module model number 15594-1 each CLIN 0003 2 Year Extended Warranty for all parts, labor, and software included. (End description). DELIVERY DATE SHIP TO ADDRESS FOB 45 Days ADC 354 MDSS/SGSLM Destination 2630 Central Ave Ste 1N07 Eielson AFB, AK 99702-2397 Prices shall be inclusive of shipping to FOB point Eielson AFB, AK. The default shipping address of Tracy, CA shall not be used. Quotes must be submitted and received no later than Monday, 25 January 2010, at 1400 hrs (2:00 P.M.) Alaska Standard Time. Quotes may be e-mailed to the attention of primary P.O.C.s listed below or faxed to 907-377-2389. The following factor shall be used to evaluate offers: low price and technical acceptability.
