- Performance Work Statement-BioHazardous Waste Pick-Up Notice Information: Combined Synopsis/Solicitation Contracting Office Address: Department of the Air Force, Air Force Material Command, 311th Air Base Group, Contracting Branch, PKBA. Bldg 723. 8150 Aeromedical Drive, Brooks City-Base TX 78235. Solicitation: Reference-Number- F1JEAB9345A002 Agency/Office: Department of the Air Force-Air Force Materiel Command Location: United States Air Force School of Aerospace Medicine (USAFSAM), Epidemiology Division, Brooks City-Base, TX 78235 Title: F108 - Hazardous Substance Removal, Cleanup and Disposal Services and Operational Support Description(s): i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. ii) The solicitation reference number is F1JEAB9345A002 and is issued as a Request for Proposal (RFP). iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-38 (effective 10 December 2009) and Defense Federal Acquisition Regulation Supplement change notice 20091123. iv) The type contract anticipated is firm-fixed price and is 100% set-aside for small businesses in accordance with Federal Acquisition Regulation Subpart 19.502-2(a). The associated NAICS code is 562112; Small Business Standard is $12.5 million v) CLIN 0001- 1 Lot- Non-Personal Services, Annual Bio-Hazardous Waste Pick-Up Services for the Epidemiology Laboratory Services Division, United School of Aerospace Medicine/PHE, Bldg. 930, Brooks City-Base, TX 78235. The attached Performance Work Statement provides a complete description of services required. vi) Estimated Period of Performance is 25 Jan 2010 through 24 Jan 2011. vii) The following Federal Acquisition Regulation (FAR) and Defense Federal Acquisition Regulation Supplement (DFARS) Clauses will apply to this acquisition: FAR 52.212-1, Instruction to Offerors-Commercial FAR 52.212-2, Evaluation-Commercial Items FAR 52.212-4, Contract Terms and Conditions-Commercial Items FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items DFARS 252.232-7003, Wide Area Work Flow Payment/Procedures viii) Contractors shall include a completed copy of FAR 52.212-3, Offerors Representation and Certifications Commercial Item. ix) Defense Priorities and Allocation System: Not applicable Technical capability will be evaluated by how well the proposed product meets the government requirement instead of predetermined sub-factors. Evaluation Basis for Award: Award will be made to the offeror who has the lowest priced, technically acceptable proposal. As well, past performance may be considered in this acquisition. It is the offeror's responsibility to be familiar with applicable clauses and provisions.
