Description
THIS IS AN AMENDMENT TO THE PREVIOUS SOURCES SOUGHT NOTICE. THIS IS NOT A REQUEST FOR PROPOSAL. The VISN 19 Rocky Mountain Network Contracting Center, 4100 E. Mississippi Ave, Glendale CO is performing market research to determine if there is a sufficient number of qualified (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small or emerging small business firms to set aside a multiple award indefinite Delivery Indefinite Quantity (IDIQ) contract. The Government will use responses to this notice to make to appropriate acquisition decision. The selected firm or firms shall provide all labor, materials, transportation, equipment and supervision, in compliance with federal, state and local regulations, to provide Custom Sterile Surgical Packs for the following VA medical facilities: Denver VAMC, Colorado, Grand Junction VAMC Colorado, Salt Lake City VAMC Utah and Cheyenne VAMC, Wyoming. The nature of this contract typically requires numerous changes to, as well as discontinuance and replacement of existing packs. Therefore, offerors submitting proposals shall be primarily engaged in the business of direct assembly of custom surgical packs for medical facilities. Offerors shall be able to demonstrate to the government that their firm has historical track record and industry expertise in the custom surgical pack business. Furthermore, the offeror shall demonstrate that, as a direct assembler of custom surgical packs, the offeror can meet the quality assurance, sterilization and environmental control standards specified in the solicitation. The offeror shall be required to submit non-sterile bid samples as part of their proposal. All interested firms who can meet the requirements stated above should respond, in writing, with a capability statement describing their interest, capabilities, geographic location and indicating the business category in which the firm belongs. The capability statement should be sent to the Contract Specialist Matt Ake no later than 3:00 p.m. (Mountain Time), December 7, 2009. Solicitation VA-259-09-RP-0371 OR CUSTOM SURGICAL PACKS has been assigned and should be referenced on all correspondence regarding this announcement. THIS IS AN AMENDMENT TO THE PREVIOUS SOURCES SOUGHT NOTICE. THIS IS NOT A REQUEST FOR PROPOSAL. NAICS Code 423450 is applicable to this acquisition, the size standard for a firm to be considered has an Emerging Small Business is no greater than 50 percent of NAICS Code with a Business Size Standard of 100 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract.
