- Description
- The George E. Wahlen VA Medical Center, Salt Lake City, Utah, in accordance with FAR Part 12 (Acquisition of Commercial Items) and Part 15 (Contracting by Negotiation), has a requirement to Provide Community Based Outpatient Care Services in the area of Juab, Sanpete and Millard Counties located in Utah. Services will be from April 1, 2010 (or date of award) through March 31, 2011, with provisions to exercise four one-year option periods. The successful Contractor shall provide and manage preventive medical care for eligible beneficiaries of the VA Medical Center. The Contractor is required to provide the medical facility, medical providers, equipment, supplies and all administrative functions for the level of care and range of services required. Two private rooms are also required for use by VA personnel. These will be for VA Patient Service Administration and Tele-psychiatry. Services will be paid at a capitated rate and will be all inclusive. Solicitation packages will be available on or about December 7, 2009. Receipt of Offerors will be on or about January 7, 2010.
