Description
DEPARTMENT OF VETERANS AFFAIRS Justification and Approval For Other Than Full and Open Competition 1.Contracting Organization: Department of Veterans Affairs, VISN 20, Puget Sound Health Care System. 2.Description of Action: This is a request for other than full and open competition for the repair/exchange on one flexible scope STORZ #11302BDD2, S/N #2050891 from Karl Storz. 3.Description of Supplies or Services: Repair service needed for one Karl Storz flexible scope that is used in surgery cases. 4.Statutory Authority: ( ) (1) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements; ( x ) (2) Unusual and Compelling Urgency; ( ) (3) Industrial Mobilization, Engineering, Developmental or Research Capability, or Expert Services; ( ) (4) International Agreement ( ) (5) Authorized or Required by Statute; ( ) (6) National Security; or ( ) (7) Public Interest 5.Validity of Use of Statutory Authority: This equipment item is used extensively to treat and care for our veterans. With a limited stock of these scopes on hand, it is imperative we get this scope repaired and returned in a few days. If not, patient care may suffer. 6.Actions to Promote Competition: Contracting Officer and Bio-Med Dept. will work together to locate source(s) that can provide adequate service and turn-around time to meet our needs during Urgent and Compelling requests. 7.Determination of Fair and Reasonable Price: The anticipated price to the Government is $ 4595.40. Previous purchase order for similar work is P92649 for $4,595.40. The Contracting Officer has determined that the costs to be fair and reasonable for this request. 8.Market Survey (FAR 10): No market research was done, due to the urgency of the repair. 9.Other Considerations: None. Surgery has a very limited amount of these scopes and a long wait time will impact patient care. 10.Listing of Sources that expressed, in writing, an interest in the acquisition: None. 11.Statement of Actions to Overcome Barriers to Competition: See #6. 12.Requirements Certification: I certify that the requirement outlined in this justification is a bonafide need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge and belief. //signed//9-21-09 _____________________________________________________ Name: David ClarkDate Title: Biomedical Engineering Technician Facility: V.A.
