This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number, VA-260-09-RQ-0667, is issued as a request for quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-33 effective July 15, 2009 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to December 2008. The North American Industry Classification System (NAICS) Code for this acquisition is 423450, Size Standard: 100 Employees. This procurement is set aside for Veteran Owned Small Businesses. Item 0001, Motorized patient transfer stretcher with battery supply, 2 EA, $_____________ System to include, but not limited To: Fowlers positions, lift assist, bucket seat, no crank knee gatch, Trendelenburg and chair positions, must not go higher than 23 from ground to reduce risk of patient falls as stretchers will be used for shorter, frail patients. 1.
