Description
The Department of Veterans Affairs (VA), Portland Veterans Affairs Medical Center has a requirement for home infusion therapy services (one (1) year base period with four one (1) year option periods). Home Infusion Therapy is provided for veterans with infections requiring long term antibiotic therapy, for veterans with cancer requiring Chemotherapy, and for pain medications, total parenteral (TPN), hydration and other non-enteral medications. This program establishes a mechanism which provides eligible patients with outpatient infusion therapy in a safe and appropriate community setting with follow up care during the course of prescribed therapy. The contractor will provide Home Infusion Therapy supplies and services, in the homes of eligible veterans and for veterans placed in community nursing homes under the direction of Portland VA Medical Center (PVAMC) Community Health Coordinator (CHC) staff. The service area includes all counties in the State of Oregon; Cowlitz, Clark, Skamania, Wahkaikum, Klickitat, and Pacific counties in Southwest Washington; and Del Norte and Siskiyou counties in Northern California. Required response time shall be within 24 hours of contractor notification from the Portland VA Medical Center (PVAMC) Community Health Coordinator (CHC) staff. This procurement is unrestricted. All bidders must be registered in CCR at http://www.ccr.gov to be eligible for award. NAICS 621610 applies-size standard is $13.5 Million. The estimated dollar value of this procurement is $8,200,000. Award will be made based on the evaluation criteria established in the solicitation.
