- US Naval Hospital Guam is Soliciting a Firm-Fixed-Price standing order type contract for the supply of laboratory test reagents. Period of performance is 13 October 2009 to 13 December 2009. Procurement is conducted in accordance with FAR 12 and 13.5. This requirement is a Small Business set-aside. Multiple delivery schedules are required per line item. Brand Name Only. There are refrigerated, perishable and shelf-life requirements on line items. Door-to-door shipping direct to Agana Heights, Guam 96910. FOB: Destination. Preservation and packaging shall afford adequate protection against damage, deterioration and corrosion during shipment from supply source. All reagents shall be packaged, palletized, crated or consolidated as a single order for overseas delivery. Reagents shall be compliant with Food and Drug Administration requirements to market and deliver a medical product for use in the United States of America, even should delivery be requested outside the United States. Responsible sources must be active in the Central Contractor Registration (CCR) and a Registered-User in the Wide-Area-Work-Flow (WAWF) Electronic Invoicing Program. Include DUNS number, Cage Code, and Tax ID Number. SOLICITATION DOCUMENT IS NOW AVAILABLE. Requests for copy of the solicitation must be in writing. Phone calls are not accepted.
