- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. Solicitation VA-244-09-RQ-0453 is issued as a request for quote (RFQ). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. The procurement is being issued as a small-business set aside under NAICS code 423460, size standard 100 employees. The Wilkes Barre VAMC requires one (1) Carl Zeiss Meditec IOL Master Version 5.4 XP, with ATS A-Plus printer with table. The items called for by this RFQ has been identified by a brand name or equal description, such identification is intended to be descriptive, but not restrictive, and is to indicate the quality and characteristics of product that will be satisfactory. Equal products (including products of the brand name manufacturer other than the one described by brand name) will be considered for award if such products are clearly identified in the offer and are determined by the Government to meet fully the salient characteristics requirements listed in the RFQ: non-contact unit, accuracy to within +/- 0.02 mm, calibrated against resolution, 40-mhz Grieshaber Biometric System, minimum of one-year warranty on the equipment and two-year warranty on the lasers, capable of fast and precise consecutive measurement of the following eye parameters in one session: axial length, corneal curvature, anterior chamber depth and optionally white-to-white. In addition, offeror must quote on a table and printer equivalent to those listed above. Only new (no used/refurbished/surplus) products with full manufacturer warranty will be accepted. Shipping will be FOB Destination 45 days ARO to Wilkes Barre VAMC, 1111 East End Blvd, Wilkes Barre, PA 18711.
