Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-09-T-0600 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 325412. Size Standard: 750.Naval Medical Center San Diego intends to solicit this requirement on a sole source basis to Cardinal Health under the authority of FAR 13.106-1(b). Quotations shall be submitted electronically by email to keely.bell@med.navy.mil. E-mail submissions are limited to 2MB. The submitter should confirm receipt of email submissions. Quotations are due on or before 25 September 2009, 10:00 AM Pacific Standard Time to be considered responsive. CLIN 0001 - Radiopharmaceuticals:Unit of Issue/ Unit Price $DMSA Vials, TC99m Neurolite, Chromiom 51, Iodine- 125, Tc99m DTPA and DTPA vials, Tc99m MAA, and MAA Vials, Pulmonlite vials, and TC99m Pulmonlite, ultratag kits, TC99m, Samarium 153, Strontium 89, hepatolite, TC99 hepatolite, Tc-99m Glucoheptonate, I-123 OIH, MDP vials and TC99m MDP, I-131 OIH, I-125 OIH, TC-99m HAM, TC-99m Sestamibi, cardiolite, myoview, 125 I HSA, TC99m Ceretec, C14 Urea, 99mTc CEA, 111 Indium Oncoscint, IN 111 Prostascint, In111 Tagged Platelets. And all other TC99m, I-131, I-123, IN-111, GA-67 pharmaceuticals and isotopes. POP 01 Oct 2009 to 31 Dec 2009.(See attached Statement of Work for additional specifications, List of Radiopharmaceuticals, and for the Delivery Schedule) Needed for Nuclear Medicine Clinic. All items listed need to be available 7 days a week 24 hours a day. All items need to be delivered directly to the NMCSD Nuclear Medicine Clinic by the shortest path available within ONE hour of request. Nuclear medicine will have the availability to phone in orders. All items will be accepted and paid for through the Wide Area Work Flow (WAWF). They need to be received by one of the nuclear medicine staff members. If items are not needed, nuclear medicine has the right to cancel or make adjustments as necessary to the orders within a 24 hour time period. Purchases are to be made by phone by a nuclear medicine technologist. Government shall have control of scheduled delivery of the product with the ability to decrease or cancel delivery when there is no need for the radiopharmaceutical on any particular day. These Isotopes will be delivered to the Nuclear Medicine Department at Naval Medical Center, Building 1, Second Floor. Vendor must be in a local area (50 mile radius) due to the half life of radiopharmaceuticals and the need to be able to receive products within the hour of notice. Under the authority of FAR 13.106-1(b), sole source justification is warranted because Cardinal Health is within a close proximity of the hospital and can supply all the pharmaceutical and radioactive pharmaceuticals and tracers to the hospital within a short period of time allowing for the Nuclear Medicine Clinic to meet daily hospital demands. Contractor must be registered to the Central Contractor Registration (CCR) prior to award. The radioactive pharmaceuticals are used in performing diagnostic nuclear scans and for treatment of patients with mild, moderate or severe health ailments of the body such as cancer. These supplies will also help to provide quality patient care and help doctors with the process of surgeries and treatment of patients. Supplies must be FDA approved and fall under NRC standards. Supplies must be delivered in a timely manner if called in the day before and within 1 hour if called on the day of appointment. All containers will be picked up with every delivery. All radiopharmaceutical products must be free of defects. All supplies must be delivered with a calibration date and time, and an experiration date and time, lot number, package insert, and a packing list. All billing will be done directly through WAWF (Wide Area Workflow) and Payment will be made for actual quantities ordered and received. All deliveries will be made directly to the Nuclear Medicine Clinic in building 1, 2nd floor via the shortest route possible.
