- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number PR-WA-09-00195 is applicable and is issued as a request for quotation. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. The requirement is "small business set-aside" with a NAICS Code of 811219 --Other Electonric and Precision Equipment Repair and Maintenance with a small business size standard of $12.0 million. PERIOD OF PERFORMANCE: Base Year plus two additional option years from the date of award. LOCATION: EPA Manchester Laboratory, 7411 Beach Dr. E, Port Orchard, WA 98366 The certification services shall be performed in the first two weeks of October every year unless otherwise authorized by the EPA. Services to be performed during normal working hours between 8:00 a.m and 4:30 p.m. pacific time, excluding federal holidays. Contractor shall conduct testing and certification of fume hoods and biological safety cabinets to provide a safe environment for laboratory personnel and in compliance with OSHA laboratory and other regulatory standards. Certification shall be performed in accordance with current NSF/ANSI 49, ANSI/ASHRAE 110, and US EPA Performance Requirements for Laboratory Fume Hoods dated February 26, 2009. This service includes HEPA filter leak tests, down flow velocity profile, inflow velocity, visual smoke test, ultraviolet lamp intensity, task lighting intensity, sound levels, and cabinet electrical safety. (NOTE: Repair of fume hoods and bio-safety cabinets is not included,) Contractor shall provide a certification tag for each Fume Hood and Bio-safety Cabinet, and a written report with a summary of Fume Hood and Bio-safety Cabinet performance and an explanation of any trends encountered. Contractor service personnel must be NSF accredited and submit their NSF accreditation certificate with the quote. Contractor shall inspect fume hoods and bio-safety cabinets to ensure conformance to applicable safety standards and effective overall performance. This service rate per unit is to include all labor, tools, equipment, and incidental materials. CLIN 0001, Yearly Certification of Fume Hoods and Bio-Safety Cabinets, Rate per Unit Price. Qty: 43 unit CLIN 0002, Yearly Optional Quantity, up to an additional 10 units, for re-testing purposes. The re-testing of Units that did not pass the first time may or may not be needed and will be requested on an as needed basis. It is not gauranteed. Rate per Unit Price. The selection resulting form this request for quotation will be made on the basis of the lowest priced technically acceptable quote. Once the Contracting Officer has decided to whom an order will be issued, a unilateral order will be issued using an Optional Form 347. A purchase order will be in existence once the Contracting Officer has signed the order. Wherever the words offer, proposal, offerors, or similar terms are used in this solicitation or in any applicable provision, they shall be read to mean quote, quotation, quoter, vendor or similar corresponding term to reflect that this solicitation is a Request for Quotation not a Request for Proposal or an Invitation for bid. Provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The Contracting Officer will evaluate quotes on the basis of information furnished by the quoter that demonstrates that they meet the requirements in the Statement of Work. The Contracting Officer is not responsible for locating or obtaining any information not identified in the quote. Telephonic inquiries will not be accepted. Quotes must be emailed, mailed or faxed along with the completed representations and certifications, providing the business size, e-mail address, Dunn and Bradstreet number, CAGE code and Federal Tax ID number. In addition, quoters must be registered in the Central Contractor Registration database at www.ccr.gov to be considered for selection. Quoters are required to submit completed copies of the provisions at FAR 52.212-3 and its ALT I, Offeror Representations and Certifications-Commercial Items, with their quotes. FAR clause 52.212-4, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition.
