Description
DESCRIPTION:This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number VA-258-09-RQ-0266 is being issued as Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 2005-36. This requirement is a 100% small business set aside under NAICS Code 238990, Size Standard of $14 Million. REQUIREMENTS: The Raymond G. Murphy VA Medical Center at 1501 San Pedro Dr. SE Albuquerque, NM 87108. The contractor shall provide and install six (6) automatic bifold doors for the DECON Area of the Albuquerque VA Medical Center. The six (6) doors will be placed in two different areas of the medical center. Four (4) sets of automatic bifold doors on shall be installed on each side of the two new DECON Showers and two (2) sets of automatic bifold doors shall be installed on the exterior of the building providing access to the DECON Area. The Doors need to have a clear opening of 72" (2") and a maximum rough opening of 84" (6"). The Doors must meet the following criteria: "Overhead-concealed breakout design with header "door panel(s), jambs " electromechanical self lubricating gear box with microprocessor control " Must have all connecting hardware " Must have actuating controls "On-Off-Hold Open Switch "A 7-pin cylinder "Doors will be installed such that water will not leak through closed doors " Frames shall be aluminum finished appropriately for either shower or outdoor environment "Glazing shall be opaque and view through glazing will be obscured "Doors will be Dorma ED1200 or equal. "Weather proof 6" stainless steel "Push to Open" button engraved with handicap logo and will configure such that doors are push to open and push to close. The contractor will be required to comply with all pertinent statutory health and safety regulations and will staff projects so that there in an OSHA 30 hour trained "competent person" on the job site whenever installation is being accomplished. FAR 52.212-1, Instructions to Offerors- Commercial, applies to this acquisition. The following clause applies to this acquisition: FAR 52.212-2 Evaluation-Commercial Items. Evaluation criteria will be based on price. Evaluation may be made with or without discussions with the offeror(s). Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items with its offer. 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition and the following addenda: VAAR 852.211-77, VAAR 852.228-71. 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders-Commercial Items applies to this acquisition. OFFERS: All responsible sources that meet the VAMC requirements may respond to this solicitation by submitting a quote that provides pricing and availability. Any offer that does not meet the solicitation requirements as deemed by the Contracting Officer may be rejected as non-responsive. Questions regarding this solicitation must be submitted on or before 12:00 p.m. (PST) September 18, 2009 to Daniel Rakes by emailing to Daniel.Rakes@va.gov. All offers must provide company name, point of contact information, the VA solicitation number and the question(s) that you would like answered. No phone calls will be accepted.
