- Wage Determination Performance Work Statement Justification and Approval This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (i) The purchase request number is F5E2MG9084A001 and the solicitation is issued as a request for quotation (RFQ) FA6648-09-T-0002. (ii) This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-36. This acquisition is restricted. See attached Justification and Approval. (iii) The associated NAICS code is 332812. The size standard code is 500 employees. (iv) This requirement consists of one line item, (17) shelters. (v) The purpose of this purchase is to provide parts, labor, material, equipment, tools, transportation, personnel and supervision necessary to accomplish Aircraft Canopy Refurbishment in accordance with the attached Statement of Work. (vi) Delivery will be made to Maintenance Group, 12710 Coral Sea Blvd, FOB Destination, 120 days ARO. (vii) The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this solicitation. (viii) The provision at 52.212-2, Evaluation - Commercial Items, applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factor shall be used to evaluate offers: (1) Lowest price. The contractor certifies that it can meet delivery requirements by submitting a quotation. (ix) The clause at 52.212-3, An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (m) of this provision. (x)The clause at 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. (xi) The following FAR clauses identified at paragraph b & c of FAR 52.212-5 are considered checked and are applicable to this acquisition:(b1), (b6), (b12), (b13), (b18), (b19), (b20), (b21), (b22), (b23), (b24), (b25), (b28i), (b39), and (c1)). ( xiv )WIDE AREA WORK FLOW - RECEIPT AND ACCEPTANCE (WAWF-RA) Electronic receiving report and invoicing instructions in accordance with Defense Federal Acquisition Regulation Supplement (DFARS) 252.232-7003, use of electronic payment requests is mandatory. Use of WAWF will speed up your payment processing time and allow you to monitor your payment status online. There are no charges or fees to use WAWF. Requests for payments (invoices) must be submitted electronically. Paper invoices are no longer acceptable.You can access payment information using the DFAS web site at (htps://wawf.eb.mil). (xvii) The clause at Air Force Federal Acquisition Regulation Supplement (AFFARS) 5352.223-9000 Elimination of Use of Class I Ozone Depleting Substances (ODS), applies to this acquisition. (xviii) The clause at AFFARS 5352.223-9001 Health and Safety on Government Installations, applies to this acquisition. (xix) The clause at AFFARS 5352.242-9000, Contractor Access to Air Force Installations, When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Volume 1, The Air Force Installation Security Program, and AFI 31-501, Personnel Security Program Management, as applicable.
