Description
RECOVERY, Recovery Act Project 663-09-132 "Design Dermatology Clinic in Former ER Space" for the Department of Veterans Affairs, Puget Sound Health Care System, 1660 S Columbian Way, Seattle WA 98108. SF 330 Architect-Engineer Qualifications applications are invited from engineering and design firms to provide professional design services and construction period services. The proposed design is for the renovation of approximately 5,275 gross square feet of space in the former first floor B100 Emergency Room, at the VA Puget Sound Health Care System, Seattle Division. The design will include an adjacent corridor from the new Medical Mall area to the existing elevator lobby for elevators P-9 and P-10. The design will also include the renovation of the adjacent musculo-skeletal clinics. Design disciplines include structural, architectural, mechanical, electrical, telecommunications, cost estimating, etc. The A/E contractor must provide bid-ready design documents 135 calendar days after A/E contract award. The A/E contractor will be responsible for the proper preparation of drawings for inclusion the digital Web site at the Federal Governments Business Opportunities System website. Provide construction period services following the construction contract award based upon this design work. Evaluation and selection will be based on qualifications stated under FBO Numbered Note 24. The selected firm or organization must have significant experience in the design and renovation of large health care facilities. Applicants must be able to demonstrate the credentials of the firm and assigned staff to support the design scope. Special qualifications and selection of firms will place emphasis on design capabilities of firms with large health care facilities design work, as well as experience in construction period services. The applicable NAISC code is 541310. This acquisition for design services will be a 100% set aside for Veteran Owned Small Businesses (VOSB). This is not a Request for Proposal. A current SF 330 representing and differentiating between the personnel, experience and capabilities of all involved offices, must be included with the application for each proposed consultant's office involved.
