- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation, FA2517-09-T-5022, is being issued as a Request for Quotation (RFQ). (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-24. (iv) This procurement is a small business set aside. The NAICS code is 238330 Flooring Contractors and the size standard is $14M. (v) Contractor shall utilize the below line item structure when preparing quotes: Line Item 0001 Description: Purchase/Install Carpet for BLDG 845 Provide non-personal services for all labor, materials, tools, equipment, and other work necessary to purchase, deliver, remove existing, and install Carpet and cove base In Accordance With (IAW) this Statement of Work (SOW) and attachments 1 and 2 at Building 845, Rm. 131, Peterson AFB, Colorado. (vi) The following commercial item is requested in this solicitation: Non-personal services for Purchase/Install Carpet for Room 131 in BLDG 845 at Peterson AFB, CO IAW the attached PWS. (vii) Advanced payments are not authorized. To be performed on Peterson AFB, CO from 15 October 2009 - 1 December 2009. (viii) The following provisions and clauses apply to this acquisition: The provision at FAR 52.212-1, Instructions to Offerors/Commercial Items. All firms must be registered in Wide Area Workflow (WAWF) at https://wawf.eb.mil at time of quote submittal. DUNS, Cage Code, tax ID, terms and conditions shall be submitted with quote. All firms must be registered in the Central Contractor Registration Database at www.ccr.gov in order to be considered for award. (ix) FAR 52.212-2(a) Evaluation - Commercial Items, applies to this acquisition. Quotes will be evaluated based on lowest price technically acceptable (LPTA). (x) The provision at FAR 52.212-3, Offeror Representations and Certification-Commercial Items, is applicable to this acquisition. All vendors must be registered in Online Representations and certifications (ORCA) (https://orca.bpn.gov)) at the time of Quote submittal. (xi) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to the acquisition.
