- Floor Plan Statement of Work This is a combined synopsis/solicitation form commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation willnot be issued. The solicitation number is 246-2009-Q-0058 for Upgrade and Expansion of Passcard Entry for Lawton Indian Hospital, Lawton, Oklahoma. This soliciation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. This acquisition is 100% set-aside for small businesses. The NAIC code is 561621 and the small business size is 100 employees. The FOB point is Lawton Indian Hospital, 1515 Lawrie Tatum Road, Lawton, Oklahoma. Evaluation will be based on price and past performance. Offerors are to include a completed copy of the provision found at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items with its offer.
