- Request For Quotation H94002-09-Q-0021 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, in conjunction with FAR 13, as supplemented with additional information included in this notice. This announcement constitutes the solicitation; quotations are being requested and a written solicitation will be issued as request for quotation number H94002-09-Q-0021. This brand name or equal requirement is for a vertical carousel electric lateral file; to include equipment, delivery, installation, and warranty. See the attached solicitation/request for quotation H94002-09-Q-0021 for the specifications, clauses, and the evaluation criteria. The delivery and acceptance will be F.O.B. Destination: TRICARE Management Activity, 16401 East Centretech Parkway, Aurora, CO 80011-9066. This requirement is not a small business set-aside based on the associated NAICS Code 333922 - Conveyor and Conveying Equipment Manufacturing with a business size standard of 500 employees. The solicitation/request for quotation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36, effective on August 11, 2009. The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The Government will award a firm-fixed-price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: technical capability and price. Offerors are advised to include a completed copy of the provision 52.212-3, Offeror Representations and Certifications - Commercial Items, Alternate I, with its offer. The clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders - Commercial Items, and the additional FAR clauses cited in the clause are applicable to the acquisition. The contracting officer has determined additional FAR and DFARS clauses and contract requirement to be necessary for this acquisition and consistent with customary commercial practices. Signed and dated quotations shall be received by September 23, 2009, no later than 3:00 PM Mountain Time. Quotations received after the date and time stated may be rejected. Quotations shall be submitted by email to: rose.roach@tma.osd.mil, cc: alicia.broner.ctr@tma.osd.mil, by fax to (303) 676-3866, or by mail to TRICARE Management Activity, Attn: Ms. Rose Roach, 16401 East Centretech Parkway, Aurora, CO 80011-9066. All emails and correspondence relating to this request for quotation shall have the solicitation number indicated in the subject block.
