Description
The 96 Contracting Squadron, Eglin AFB, FL, intends to solicit, negotiate, and award a Firm-Fixed Price contract for purchase, delivery, and installation of four (4) each Hydraulic Dock Levelers and four (4) each corresponding Truck Restraints/Locks. This acquisition is under Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, and FAR Subpart 13.5, Test Program for Certain Commerical Items. This announcement constitutes the only solicitation; a quotation is being requested, and a written request for quotation will not be issued. This solicitation is issued as Request for Quotation (RFQ) F1T3BU9058A002. The North American Industry Classification System (NAICS) for this acquisition is 333924, small business size standard: 750. Please identify your business size in your response based upon this standard. This is a Full and Open competition. CLIN requirements are: CLIN 0001: Four (4) each Hydraulic Dock Levelers, Capacity 40K, Two Steel Faced Dock Bumpers CLIN 0002: Four (4) each Truck Restraints; to fit/interface with items in CLIN0001. CLIN 0003: Labor For Installation for CLINs 0001 & 0002 The following is the Statement of Work Requirements: Vendor shall acomplish installation and performance test of new dock levelers and truck restraints. The new dock levelers and truck restraints must form, fit, and function in the existing concrete cut outs where the present dock levelers are located. They must use existing electrical supply for install and operation. The present electrical circuit is 230 volt, 20 amp, 3 phase. The current installed levelers are to be replaced are McGuire Autodok Levelers model number 117308, rated at 220 volt, 40 amp. Any new dock levelers to be offered must have a capacity of No Less Than 40,000 Lbs. The existing diamond plate steel deck is ¾" thick by 83" wide and 83" long with a 16" x 83" lip. The present cutout is 85" x 89" x 24". Corresponding Dock Locks/Truck Restraints are required; these are safety features that allow vehicles to be locked in place/to keep them from rolling, once the vehicles are backed up to the docks. Contractor will remove and dispose of old dock levelers. A site visit is scheduled for 18 September 10:00AM CST, at 96 LRS, 108 E Eglin Blvd, Bldg 600, Eglin AFB, FL 32542. You must contact David Serbe (see POC instructions at end) for coordination of advance security/base pass requirements. Site Visit Security Requirements: Interested parties must contact David Serbe, david.serbe@eglin.af.mil, for base access security requirements prior to site visit, and for site visit information. Dock Levelers Installation Work: Contractor shall comply with Eglin AFB security requirements in order to obtain base passes for the work period. Contractor worker affidavit forms must be filled out and submitted to Eglin AFB 96th Security Forces, at least three days prior to anticipated site visit date. A base pass request letter must also be sent to 96th Contracting Squadron, naming the workers, their social security numbers, and various contract information. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or security police for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and any other required information to obtain a vehicle pass. All deliveries to contractor work sites on Eglin AFB main base must go through the North Gate, located off SR-85 South. Trucks trying to enter the East, West or the ACC gate will be turned around and told to use the North Gate. Contractors' work trucks carrying equipment and tools are also required to use the North Gate. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with [insert any additional requirements to comply with AFI 31-101, Volume 1, The Air Force Installation Security Program, and AFI 31-501, Personnel Security Program Management,] citing the appropriate paragraphs as applicable. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment. (End of clause) 5352.223-9001 - HEALTH AND SAFETY ON GOVERNMENT INSTALLATIONS (JUN 1997) (a) In performing work under this contract on a Government installation, the contractor shall: (1) Comply with the specific health and safety requirements established by this contract; (2) Comply with the health and safety rules of the Government installation that concern related activities not directly addressed in this contract; (3) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and (4) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes. (c) The contracting officer may, by written order, direct Air Force Occupational Safety and Health Standards (AFOSH) and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract. (d) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.
