- This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications --Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; and 52.212-5 Contract terms and Conditions to Implement Statutes or Executive Orders - Commercial Items. NAICS for this requirement is 423850. The U.S. Naval Hospital, Okinawa, Japan has an requirement for CLIN 0001:(LD500)Ventilated Tissue Storage System. MOPEC Inc., UNIT PRICE ___________ QUANTITY: 6, TOTAL PRICE___________ SPECS: All stainless Steel cabinet construction, door to the cabinet is clear heavy duty lexan and may be hinged on the left or right. Includes storage cart (LD501) 67-1/2”H x 24-1/2”W x 24-1/2”D and overall dimentions are 34”L x 30”D x 85”H. During transport, smaller containers are conveniently located in trays which provide spill proof containment and a means of absorbing and neutralizing formalin. The contractor shall be responsible for delivery to Tracy, CA 45-60 days ARO and providing proof of delivery and tracking information (tracking number) shall be provided to the Government upon request. Contractors shall re-ship orders, at their expense, if proof of delivery and proper tracking information cannot be provided. This notice of intent is not a request for competitive quotes; however, any firm that believes it can meet the requirements may give written notification to the Contracting Officer within 10 calendar days Sep 25, 11:00 AM Japan Standard Time: from the date of publication of this synopsis. Supporting evidence must be furnished in sufficient detail, to demonstrate the contractor’s capability to provide and ability to comply with the above requirements. Information can be e-mailed to the Contracting Officer. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technical capability of the item offered to meet the Government requirement (including submission of descriptive literature); (2) Price; all to be submitted with quotation. No telephone requests will be accepted. Responses received will be evaluated, however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. Faxed quotes will be accepted. Contractors are responsible for obtaining any and all amendments or additional information.
