- *AMERICAN RECOVERY AND REINVESTMENT ACT* The Indian Health Service has a requirement to purchase an Optometry Retinal Fundus Camera with Digital Camera Interface, Interfacing Laptop Computer, and Adjustable Power Stand. This retinal camera will be delivered FOB destination to the Pawhuska Indian Health Center located at 715 Grandview, Pawhuska OK 74056. The solicitation will be issued as Request for Quotes (RFQ) 246-09-Q-0062. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This requirement is being procured in accordance with FAR Part 12, Acquisition of Commercial Items. A firm, fixed-price commercial order will be issued. Vendor must have an updated CCR and ORCA record by the solicitation deadline to be considered for the award. The closing date for receipt of quotes is September 21 2009. Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 4:30 p.m., on September 21 2009. The offer must be submitted in a sealed envelope, addressed to this office, showing the solicitation number, and your name and address. Please do not email RFQ's. Specifications of Equipment: Camera MUST have ability for anterior and posterior imaging of the eye Camera MUST have 20 Megapixel or greater for better resolution capability Digital camera with Auto-alignment., 3D-tracking, auto-focus Automatic Stereo Full frame 36mm x 24mm sensor
