- Combined Synopsis/Solicitation W81K00-09-T-0590 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. This acquisition is solicited for small business set-aside for NAICS 325413, size standard 500 employees. All responsible sources may submit a quotation which shall be considered by the agency. The requirements of this solicitation is for 2 cost per test analyzers and various annual reagent testing kits for Darnall ACH Fort Hood, TX. Required items to be delivered FOB destination. ITEM 1: AMAX DESTINY COAGULATION ANALYZERS, or equal, 2 each ITEM 2: SIMPLASTIN EXCEL S, #T1104, or equal, 25 each ITEM 3: PLATELIN L w/CaC12, #BX259573, or equal, 19 each ITEM 4: VERIFY LEVEL 1, #T4111, or equal, 37 each ITEM 5: VERIFY LEVEL 3, #T4113, or equal, 37 each ITEM 6: AMAX DESTINY REACTION TRAYS, #Z04000, or equal, 12 each ITEM 7: AMAX DESTINY PROBE WASH, #DPW10, or equal, 10 each ITEM 8: AMAX FIBRINOGEN KIT, #T1301, or equal, 12 each ITEM 9: FIBRINOGEN CONTROL LOW, #T4105, or equal, 32 each Salient characteristics for all line items excluding ITEM 1: Must equal chemical composition of items as indicated by name and part number above for testing kits. If offering brand name or equal, demonstrate how item is equal to listed item(s). The following FAR and DFARS provisions and clauses apply to this solicitation: 52.212-1 [Instructions to Offerors]; Addendum 52.212-1; Federal Acquisition Regulation (FAR) provision 52.211-6 Brand Name or Equal applies to this acquisition. Vendors shall comply with FAR provision 52.211-6 if submitting other than the brand name products. Award will be all or none. The closing date and time of this solicitation is 18 September 2009, 10:00 a.m. CST. The Government will award a contract resulting from this solicitation to the responsible contractor whose quote, conforming to the solicitation, is Lowest Priced, Technically Acceptable (LPTA). Award will be on an all or none basis. The following factors will be used to evaluate quotes: Technical acceptability and price. 1. Technical acceptability: If providing an equal item(s), comply with FAR Clause 52.211-6, Brand Name or Equal. Explain how your company will meet the salient characteristics of the items as specified in the solicitation. Prospective contractors shall demonstrate that the items offered complies with the technical requirements described in the salient characteristics by submission of a written capability statement with their offer. 2.
