Description
This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Request for Quotation (RFQ) number is VA-246-09-RP-0237. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2000-07. This is 100% set-aside for Service Disabled Veteran Owned Small Business. NAICS code is 238320. Size standard is $14.0 Million. The VA Medical Center, Fayetteville, North Carolina intends to award a firm fixed price contract for maintenance in Building 4, 2nd Floor as follows : The Contractor shall provide all supervision, labor, materials and equipment necessary to install a refrigerator, stove, microwave oven, and dishwasher in the kitchen area and appliances must be Energy Star; remove approximately 12 old door locks to be returned to facility locksmith and install new locks on select doors; replace all carpet flooring, paint walls, to include repair and patch as necessary on walls, floors, doors, and windows; replace those items that are beyond repair in the hallways, bedrooms, bathrooms, conference room, or any other area on the 2nd floor of Building 4 within the VA Medical Facility at Fayetteville. Performance of work will meet all State, Federal & VA regulations and specifications as indicated in the VA national specifications. The Contractor shall provide all supervision, labor, materials and equipment necessary to install a refrigerator, stove, microwave oven, and dishwasher in the kitchen area (all electrical Energy Star appliances); remove old door locks (return to medical center locksmith) and install new locks on select doors (approx. 12 locks); replace all carpet flooring, paint walls, and repair and/or patch (as needed) walls, floors, doors, and windows; replace those items that are beyond repair in the hallways, bedrooms, bathrooms, conference room, or any other area on the 2nd floor of Building 4 within the VA Medical Facility at Fayetteville. Performance of work will meet all State, Federal & VA regulations and specifications as indicated in the VA national specifications. Specific duties will include but are not limited to the following: Sign in and pick up work badges prior to commencing any work. Cover all office equipment and furniture that cannot be moved prior to commencing any work. Remove and dispose of wall coverings in hallway. Remove and return to VA Locksmith door locks and hardware as not to damage existing structures to the following rooms: 201, 202, 203, 206, 207, 208, 213, 215, 216, 217, 218, and 219 (main entrance doors only.) Purchase and install new Falcon LM571 Dormitory door locks with Dane-Napa handles to the following rooms: 201, 202, 203, 206, 207, 208, 213, 215, 216, 217, 218, and 219 (main entrance doors only.) Purchase and install the following Energy Star electrical kitchen appliances (black stainless steel): a 68 refrigerator, 30 slide in range with smooth cook top, 24 dishwasher with adjustable legs, and a 2 cu. ft. countertop microwave oven. OPTIONAL ITEM: Purchase and install a standard/residential black Energy Star washer and dryer (status will be known at the time of award.) Demolish, remove and dispose of carpet & cove base in the hallway, bedrooms, conference room and offices. Patch, repair, level, and prep floors and install new carpet and 6 cove base in hallway, bedrooms, conference room, and offices in accordance with VA standards. Patch, repair and/or replace if needed: walls, trim, door trim, window trim, door casing, etc. in accordance with VA standards. Doors and door frame areas MAY CONTAIN LEAD BASED PAINT AT THE LOWEST POSSIBLE LAYER OF PAINT. USE PRECAUTION WHEN PREPARING THESE AREAS AS NOT TO DISTURB THE PAINT AT THE LOWEST LEVEL. IT IS RECOMMENDED THAT THE DOORS AND DOOR FRAME AREAS BE PAINTED OVER IN ORDER TO AVOID LEAD ABATEMENT PROCEDURES. REPORT ANY INCIDENTS AND/OR PROBLEMS IMMEDIATELY. Do not remove or paint blue signage and/or some safety signs. Before painting & patching, remove all artwork, bulletin boards, plaques, posters, magazine holders, paper towel and soap dispensers, coat hangers, mail boxes, old signs (brown in color), etc. and store in secure location where they will not be damaged. Reinstall after patching and painting is complete in the area. Prep, patch and paint: two exit doors, all walls in the hallway, bedrooms, bathrooms, conference room, and offices in accordance with VA specifications. NO PAINTING IS NEEDED IN THE FOLLOWING ROOMS: 202, 203, 212, and 213. Prep, patch and paint already painted: trim, door trim, window trim and casing, door casing, electrical panels, circuit panels, fire equipment doors, doors, radiators, etc. in accordance with VA standards. NO PAINTING IS NEEDED IN THE FOLLOWING ROOMS: 202, 203, 212, and 213. Construct a non-flammable barrier during all phases of work, as deemed necessary by COTR. All work will be performed between the hours of 8:00am and 5:00pm on weekdays. Repairs must be accomplished by a licensed contractor. Evaluation will be in accordance with FAR 52.212-2 Evaluation--Commercial Items. A site visit is scheduled for Wednesday, 15 Sep 2009 at 1:00 pm and all offerors are encouraged to attend.
