- Description
- Home Infusion and Skilled-Care Services: Provide all supervision, labor, drugs and transportation to coordinate and perform infusion therapies and skilled-nurse care to eligible beneficiaries located within the Baypines VAHCS catchment area. All services provided by the prime and its subcontractors shall meet or exceed all current Home Care TJC, Community Health Accreditation Program (CHAP), and/or Alliance of Coalitions for Healthy Communities (ACHC) standards, Center for Disease Control (CDC) guidelines, and standards for equipment management and Federal Drug Administration (FDA) requirements. In addition, the contractor shall comply with all Department of Transportation (DOT) and FDA regulations. This procurement will result in a Requirements contract for one base year and four additional one year periods and is being issued on an unrestricted basis. The request for proposal will be issued on or about October 15, 2009 and the anticipated award date is December 15, 2009. The solicitation will be posted at this website. The North American Industrial Classification System code is NAICS-621610. By submitting an offer, the offeror acknowledges the requirement to be registered in the Central Contractor Registration (CCR) database (www.ccr.gov) prior to award of any contract.
