Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number N68836-09-T-0375 is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the lastest edition of the Federal Acquisition Circular. This solicitation is issued on an Total Small Business Set-A-Side basis and/or Non-Profit Organization (see Numbered Note 1), NAICS Code 711110; (SEE ATTACHED STATEMENT OF WORK) STATEMENT OF WORK FOR RETREATS Services for the delivery of four Marriage Enrichment Retreats (MER), two Personal Growth Retreats (PGR), one Family Enrichment Retreat (FER), and a to be determined (TBD) amount of Team Building Workshops (TBW) for a total of at least seven retreat programs from 01 October 2009 to 30 September 2010 conducted by the Spiritual Fitness Division (SFD) Southeast formerly known as Chaplains Religious Enrichment Development Operation (CREDO) Southeast and sponsored by the Chief of Chaplains (N097A2). BACKGROUND SFD Southeast provides specialized forms of ministry and support services for Active Duty, Reserve and retired members of the Navy, Marine Corps and Coast Guard and eligible family members. These sea service personnel are enabled to develop personal and spiritual resources and grow toward increased functional ability, religious maturity and acceptance of responsibility. Personnel are helped to resolve personal and professional concerns so as to be of greater useful service in their military occupations. The primary form of SFD ministry uses a retreat format for sea service and Marine Corps personnel and their eligible family members. In order to remove people from the pressures and entanglements of daily life, these retreats are held at civilian sites. Marriage Enrichment Retreats are conducted from Friday afternoon (48 hour retreats) through Sunday afternoon. SPECIFICATIONS 1.0 General. 1.1 The retreat site must be located within two hours commuting time of NAS Jackonville, Florida. 1.2 The retreat site must provide privacy for participants dealing with personal, confidential issues. 1.3 The retreat site must have staff personnel available throughout the retreat who are authorized to take immediate action to resolve site problems. 1.4 A telephone shall be available at the retreat site for receiving incoming calls and making outgoing calls limited to the local area as approved by the Retreat Leader. 1.5 All retreat facilities used by SFD Southeast must be clean and neat upon arrival. 1.6 Heating and ventilation must have the capacity to maintain temperature between 68 and 75 degrees in all retreat facilities used by SFD Southeast. 1.7 The retreat site shall have year round inside and or outside recreational facilities and equipment. 1.8 Transportation by automobile or van must be available at the retreat site in case of medical emergency. The contractor will brief the Retreat Leader on resources for use in an emergency situation, such as fire extinguishers, first aid devices, etc. 1.9 Contractor shall comply with all state and local fire and sanitation regulations. 1.10 Smoking shall be allowed in designated areas adjacent to the meeting rooms used by SFD Southeast. Contractor shall provide receptacles for smoking materials. 1.11 Contractor shall provide notification (verbal or written) two weeks in advance of any disruption of retreat facilities or retreat atmosphere. The Acting Director, LCDR Kim M. House, CHC, USNR, may be contacted at (904) 542-5645. 2.0 Berthing. 2.1 MER/FER requires private rooms provided for each individual/couple/family. Queen beds are preferred but at a minimum provide two double beds (or one double bed and one single bed). 2.2 Separate toilet facilities and bathing/showering facilities for each room maintained in sanitary and good working condition. 2.3 Berthing spaces shall be furnished with blankets, linens and towels. Towels shall be changed daily. 2.4 Rooms shall be non-smoking only. 3.0 Meeting Rooms. 3.1 Plenary meeting room with minimum 1000 square feet so that up to 100 people can set in it comfortably. See diagram (page 7) for the details. 3.2 Meeting room must be self-contained and separate from any other personnel who may be using the retreat site and provide freedom from outside distraction or interference during the retreat process. 3.3 Provide portable chairs for the plenary meeting room: MER, 32; FER, 100; SGR, 20; and a PGR, 12 chairs. 3.4 Provide TV/VCR set up of adequate size to be seen by all the participants. 3.5 Provide three portable tables (approximate size three feet by six feet). 3.6 Provide window coverings, either curtains or blinds, for relief from outside distractions. 3.7 SFD Southeast furnishes and plays recorded music throughout the retreat. 3.8 Separate toilet facilities for males and females maintained in sanitary and good working condition shall be provided. These facilities must be located within a one-minute walking distance of the plenary meeting room. 3.9 Two separate breakout meeting rooms with a minimum of 500 square feet floor area each reserved for small group of 50 people to sit in a circle. 3.10 These rooms must be adjacent to the plenary room or within a one walking distance. 4.0 Beverages and Snacks.(see vendor menus for specifics) 4.1 Coffee (both regular and decaffeinated), tea, hot chocolate, cold juice and water shall be available at all times. Sugar, a sugar substitute, cream and a cholesterol free cream substitute shall be available. 4.2 The contractor shall provide cups, napkins and stir sticks. 4.3 Snacks shall be available according to the type of retreat program listed below. The retreat program discourages use of alcohol but charges for alcohol are the responsibility of the individual purchasing it. 5.8 On full session days, three balanced meals shall be required, with the exception of the TBW. 5.9 Sample Breakfast Menu:Juice Milk (Regular and Low Fat) Cold CerealToast (White and Wheat) Eggs and Meat Entree (Bacon, Ham or Sausage) or alternate of Waffles, Pancakes or French Toast with Syrup Potatoes Fruit (Fresh, Frozen or Canned) Butter and Butter Substitute Jam, Jelly, Honey, Catsup, Salt, Pepper 5.10 Sample Lunch Menu. Gift baskets placed in the participant's room prior to arrival. 6.0 Retreat Contract Notes. 6.1 Rental facilities are required for seven retreats to be conducted on: 23-25 Oct, 11-13 Dec, 15-17 Jan 2010, 5-7 Feb, 26-28 Mar, 16-18 Apr, 21-23 May, other dates TBD for conducting TBW using the confidence course facilitated by Peak Pursuits. 6.2 The following is a list of maximum number of people for each event: MER = 32; PGR = 10; FER = 85; SGR 15; and TWR = 25 participants each. 6.3 The Government may cancel the scheduled retreat session at no cost providing that the cancellation notice is given not less than 60 days prior to the scheduled arrival time. If cancellation is required with less than 60 days notice, the Government may be accessed a $250.00 cancellation charge. This charge will not apply in the case of Acts of God or emergency military operations with a 24 hour notice prior to the scheduled arrival time. 6.4 The government will be allowed 10% shrinkage without cost based on the attendance shown in paragraph 6.2 above. The government may be accessed a charge of $10.00 per person per night reserved for any reduction in number greater than this 10% allowance. 6.5 Retreat Schedule: see section 6.1. 6.6 Child care services for the MER. Licensed child care services that work with Epworth by the sea shall provide childcare for up to 25 children at $ 90.00 per child for up to 20 hours during the FER 7.0 Designation of Government Point of Contact. 7.1 The Government Point of Contact for technical questions for this retreat is: Kevin C.
